Analysis of Hospitalisation Costs of Patients with Cerebral Infarction in a Grade-A Tertiary Hospital in Shanxi Province Before and After DRG Implementation
Objective:To explore the change of hospitalization cost of cerebral infarction patients before and after the imple-mentation of DRG in a grade-A tertiary hospital in Shanxi Province,and to evaluate the cost-control effect of DRG payment,so as to provide reference for optimizing the structure of medical expenses and hospital fine management.Methods:The information of inpatients with cerebral infarction in a grade-A tertiary hospital in Shanxi Province from 2019 to 2022 was collected.The propensity score matching method was used to balance the differences between the two groups before and after the implementation of DRG pay-ment,and the hospitalization costs of the two groups were compared and analyzed.The grey correlation method was used to assess the correlation degree of internal factors of hospitalization costs,and the structural change analysis method was used to evaluate the characteristics of changes in the structure of hospitalization costs for cerebral infarction.Results:Compared with those before DRG payment,the average total hospitalization cost and each item cost of cerebral infarction patients after DRG payment were significant-ly decreased,and the differences were statistically significant(P<0.05).Among the cost categories of cerebral infarction patients,diagnosis cost(0.995)had the largest correlation,followed by drug cost(0.986)and comprehensive medical service fee(0.816).From 2019 to 2022,the change degree of the hospitalization cost structure was 17.24%,and the top three costs that contributed to the change in structure were the comprehensive medical service fee(40.20%),diagnosis fee(35.56%)and drug fee(10.73%).Conclusion:DRG payment can effectively manage the overall hospitalization expenses and different hospitalization expenses of pa-tients with cerebral infarction,and promote the reasonable change of the cost structure,but the cost structure still needs to be im-proved.DRG payment can reduce the economic burden of patients and standardize the diagnosis and treatment behavior of doctors.DRG payment reform should be continuously promoted and improved.
