摘要
目的:了解北京市某三甲医院乳腺癌患者术后病耻感现状,并探讨其与自我同情的关系,为乳腺癌患者术后病耻感的干预提供科学依据.方法:选取北京市某三甲医院2022 年8-12 月收治的273 例乳腺癌患者为研究对象,采用一般资料调查表、社会影响量表、自我同情量表对其进行调查.采用分层回归分析探讨乳腺癌患者术后病耻感与自我同情的关系.结果:共回收有效问卷 272 份,乳腺癌患者术后病耻感总分为51.56±11.74 分,不同居住地、手术方式、癌症分期、是否接受新辅助化疗、以及不同癌性疼痛程度的患者术后病耻感具有显著差异(P<0.05),术后病耻感得分与其自我友善、自我批判、孤立感、正念、过度认同维度及自我同情总分呈负相关关系(P<0.01).分层回归分析显示,自我同情是乳腺癌患者术后病耻感的影响因素(P<0.01),可独立解释术后病耻感12.2%的变异(R2=0.122,P<0.001).结论:乳腺癌患者术后病耻感处于中等程度,自我同情是乳腺癌患者术后病耻感的影响因素,可通过提高乳腺癌患者的自我同情以减轻其术后病耻感.
Abstract
Objective:To investigate the correlation of postoperative stigma in breast cancer patients in a grade-A tertiary hospital in Beijing with self-compassion,so as to provide the scientific basis for the intervention of stigma in breast cancer patients.Methods:A cross-sectional study was conducted,involving 273 breast cancer patients admitted to the Department of Breast Sur-gery at a third-grade hospital in Beijing between August 2022 and December 2022.Participants completed a general data question-naire,a social impact scale,and a self-compassion scale.Stratified regression analysis was performed to explore the relationship between self-compassion and self-stigma in breast cancer patients.A total of 273 questionnaires were distributed,and 272 valid questionnaires were collected,yielding an effective recovery rate of 99.7%.Results:The mean total score on the stigma scale for the 272 breast cancer patients was 51.56±11.74.Significant differences in stigma were observed among patients with varying places of resi-dence,cancer stages,type of surgery,preoperative neoadjuvant chemotherapy status,and cancer-related pain(P<0.05).Stigma scores showed a negative correlation with total scores for self-kindness,self-criticism,isolation,mindfulness,over-identification,and self-compassion(P<0.01).Stratified regression analysis revealed that self-compassion significantly influenced stigma in breast cancer patients(P<0.01),accounting for an independent 12.2%variation in stigma(R2=0.122,P<0.001).Conclusion:Breast cancer patients exhibit a moderate level of stigma,with self-compassion identified as a significant factor influencing self-stigma.Nursing interventions to enhance breast cancer patients'self-compassion levels can mitigate stigma.
