Study on Influencing Factors of Hospitalization Costs and Days for Patients with Cardiovascular and Cerebrovascular Diseases in Sichuan Province Under DRG Payment
Objective:To investigate the influencing factors of hospitalization cost and number of days for patients with cardio-vascular and cerebrovascular diseases in Sichuan province under DRG payment,and to provide references for the reasonable control of hospitalization cost and number of days by health insurance departments and hospitals.Methods:By using univariate analysis and multilevel multivariate model,the differences in hospitalization costs,days of hospitalization,and influencing factors among pa-tients with cardiovascular and cerebrovascular diseases in Sichuan province from January 2020 to December 2021 were analyzed,and the degree of impact was compared.Results:After the implementation of DRG,the cost of hospitalization for patients de-creased by 1.68%and the number of days in hospital decreased by 4.42%(P<0.05),and DRG payment affected the number of days in hospital to a greater extent compared to the cost of hospitalization.In addition,the nature of the hospital,gender of the pa-tient,age,grade of surgery,comorbidities and complications,and type of case were all important factors affecting the cost and number of days(P<0.05),with specialty hospitals and gender mainly affecting the number of days of hospitalization(P<0.05),and general hospitals,age,high-multiplicity cases,and grade of surgery mainly affecting the cost of hospitalization(P<0.05).Conclusion:The hospitalization cost and number of days for patients with cardiovascular and cerebrovascular diseases in Sichuan province is under control under DRG payment,and there are differences in the degree of influence of influencing factors at each lev-el of hospitals and patients on the hospitalization costs and days of patients with cardiovascular and cerebrovascular diseases,sug-gesting that the health insurance department should evaluate hospitals by the nature of hospitals,and also suggesting that hospitals should adopt different measures when managing hospitalization costs and days of hospitalization within the hospitals.
