Health management is a key focus of chronic disease prevention and control in China,but there are still many is-sues that need to be addressed in evaluation systems,management models,and other aspects.The sense of gain of patients with chronic diseases in health management is an important indicator to test the effectiveness of health management and a key point in the transformation of health management modes.Based on the theoretical foundations of need theory,cognitive differences,and the ac-tor-observer effect,explore the conceptual connotations of the sense of gain in health management among chronic disease patients.Further analyze the main constraints in enhancing the sense of gain in health management among chronic disease patients.The cur-rent health management evaluation system is not yet perfect,and the assessment direction deviates from the patients'sense of gain.Chronic disease patients have low proactive health awareness,and the phenomenon of"dulling"sense of gain is prominent.Addi-tionally,there is a lack of resources to improve the sense of gain in health management for chronic disease patients.Based on these findings,we put forward countermeasures and suggestions to improve the sense of gain in health management for chronic disease pa-tients at the grassroots level.We need to establish and improve a health management evaluation system with the sense of gain at its core,make full use of modern information technology to enhance the sense of gain from multiple perspectives,and comprehensively implement traditional Chinese medicine at the grassroots level to promote its application in grassroots health management.
