摘要
目的 探讨CT检查对小儿肺炎支原体肺炎(MPP)的诊断价值.方法 选取收治的 100 例疑似MPP患儿,均行病原学和CT检查,将病原学检查结果作为金标准,统计CT检查对MPP的诊断效能.结果 病原学检查发现阳性 88例,阴性 12 例;CT检查发现阳性 86 例,阴性 14 例.CT检查的诊断准确度、敏感度和特异度分别是 96.00%、96.59%、91.67%.100 例中 86 例CT检查结果为阳性的患儿,病灶处于单侧肺叶病变占比是 88.37%;两侧肺叶病变占比是11.63%,分布在右上肺叶、右中肺叶、右下肺叶、左上肺叶、左下肺叶占比分别是 29.07%、26.74%、34.88%、11.63%、9.30%.病变位置的CT检查显示大面积斑片样阴影、斑点样阴影、肺部纹理增多、条索样阴影及磨玻璃样阴影,占比分别是 87.21%、38.37%、33.72%、19.77%、15.12%.结论 CT检查能清晰显示MPP患儿肺内病变情况.
Abstract
Objective To analyze the diagnostic value of CT for mycoplasma pneumoniae pneumonia(MPP)in children.Methods A retrospective analysis was conducted on the clinical data of 100 suspected MPP patients admitted to our hospital from June 2020 to November 2022.Pathogenic tests and CT examinations were conducted,and the final results of the pathogenic tests were used as the gold standard to evaluate the diagnostic efficacy of CT examinations for MPP.At the same time,the manifes-tations of the patients during CT examinations and changes in CT images before and after treatment were summarized.Results Pathogenic examination found 88 positive cases and 12 negative cases;CT examination found 86 cases positive and 14 cases negative.The diagnostic accuracy,sensitivity,and specificity of CT examination were 96.00%,96.59%,and 91.67%respectively.Among 86 patients with positive CT results,88.37%had unilateral lobar lesions in terms of lesion distribution;The proportion of bilateral lung lobe lesions was 11.63%.The proportion distributed in the right upper lobe,right middle lobe,right lower lobe,left upper lobe,and left lower lobe was 29.07%,26.74%,34.88%,11.63%and 9.30%respectively.The main CT features of the lesion location were extensive patchy shadows,patchy shadows,increased lung markings,striped shadows,and ground glass shadows,ac-counting for 87.21%,38.37%,33.72%,19.77%,and 15.12%respectively;In terms of other manifestations of the lungs,the thick-ening of bronchial tube walls,enlargement of hilar and mediastinal lymph nodes,and pleural effusion accounting for 79.07%,11.63%and 11.63%respectively.Conclusion CT examination can clearly present the pulmonary lesions in children with MPP,which has a high diagnostic efficiency and can provide important reference for evaluating the condition of the child.
