摘要

目的 探讨乳腺MRI导管异常信号作为伴随征象的鉴别诊断意义.方法 选取经病理证实的乳腺纤维腺瘤及(或)腺病、肉芽肿性小叶性乳腺炎(granulomatous lobular mastitis,GLM)、乳腺癌(breast cancer,BC)共 172 例患者,将172 例患者按三类疾病分为三组;乳腺纤维腺瘤及(或)腺病组、GLM组、BC组,对预扫描导管高信号及导管样强化在三组数据的阳性率进行卡方检验,判断差异是否有统计学意义.多元logistic回归分析导管样异常信号对鉴别三类病变的价值.结果 预扫描导管高信号三组阳性率差异无统计学意义(P>0.05),导管样强化三组阳性率差异有统计学意义(P<0.05).多元logistic回归分析:以纤维腺瘤及(或)腺病组为对照,导管样强化增加GLM诊断的可能性差异有统计学意义(OR=10.011,95%CI 3.492-28.703,P<0.001).以BC组为对照,导管样强化增加GLM诊断的可能性差异有统计学意义(OR=4.435,95%CI 1.772-11.096,P=0.001).结论 导管样强化对GLM诊断具有一定价值.

Abstract

Objective To investigate the value of duct abnormal signal among MR imaging-detected breast lesions biop-sied.Methods Retrospective review was performed on 172 lesions that had breast MR imaging and pathological results.Ac-cording to the pathological results,they were divided into three groups,i.e.,groups of fibroadenoma and adenosis,granuloma-tous lobular mastitis,and breast cancer.T1 pre-scan duct high signal and ductal enhancement were reviewed and recorded.The positive rates of the three groups were observed and subjected to statistics analysis using the chi-square test.T1 pre-scan high sig-nal and ductal enhancement were optimized for three groups using logistic regression analysis.Results For fibroadenoma and adenosis,GLM,and BC differentiation,ductal enhancement was a key factor for predicting granulomatous,wherein,OR was 10.0 for fibroadenoma and adenosis,and OR was 4.4 for breast cancer(P<0.05).Conclusion Ductal enhancement are predic-tive of GLM.
