摘要

目的 分析胃肝样腺癌的 18F-FDG PET/CT显像特征,结合血清AFP水平,两者联合提高对该病认识及诊断准确率.方法 选取 5 例经病理学证实胃肝样腺癌患者的 18F-FDG PET/CT检查资料,分析患者的临床表现、原发病灶位置及形态、最大标准摄取值(SUVmax)、肿瘤标志物水平、预后情况.结果 原发病灶位于胃窦 3 例,位于贲门-胃体处 2例.5 例病灶的SUVmax范围为 7.58~23.6,平均值 13.8±6.4.5 例中位生存期为 8 个月.结论 胃肝样腺癌是一种特殊类型癌,侵袭性强,临床表现无特异性,明确诊断有赖于影像学与病理结合,无肝脏病变时患者血清AFP升高具有重要的诊断提示意义.

Abstract

Objective To improve the knowledge and diagnostic accuracy of the hepatoid adenocarcinoma of the stomach via-investigating the characteristics of 18F-FDG PET/CT imaging of this disease in combination with the level of the serum AFP Methods The 18F-FDG PET/CT data of 5 patients with hepatoid adenocarcinoma of the stomach confirmed by pathology were collected,and the clinical manifestations,location and shapes of primary focus and maximum standard uptake value(SUVmax),the level of the tumor markers and the prognosis were analyzed.Results Three cases were located in gastric antrum,two cases in gastric cardia-body.The range of SUVmax in five cases was 7.58 to 23.6,and the average value was(13.8±6.4).The median survival time was 8 months.Conclusion As a rare special type of cancer with no special symptoms,hepatoid adeno-carcinoma of the stomach has strong invasiveness.By combining the imaging and pathology,we can often make the diagnosis of the hepatoid adenocarcinoma of the stomach.If patients have no diseases of the liver,the increase of serum AFP is an important diagnostic clue.
