摘要
目的 探讨影响布加综合征(BCS)患者肝脏磁共振弹性成像(MRE)图像质量的主要因素.方法 选取 59 例经DSA确诊的布加综合征患者.所有患者均行MRE检查,由 2 位放射科医师独立分析MRE图像质量,并将其分为高质量组和低质量组.纳入分析的影响因素包括性别、体重、体重指数(BMI)、肝脏脂肪含量、肝脏铁含量、腹水、腹壁侧支循环.利用Kappa检验分析 2 位医师对MRE图像质量评价的一致性,利用单因素分析及logistic回归分析明确影响BCS肝脏磁MRE的决定性因素.结果 观察者之间对MRE图像质量评价具有高度的一致性(Kappa =0.872).单因素分析结果表明BMI、肝脏脂肪分数、肝脏铁沉积、腹水和腹壁静脉曲张组间比较具有差异性.多因素logistic回归分析提示BMI(P=0.015,OR=1.888)、腹水(P=0.001,OR=7.100)和腹壁静脉曲张(P=0.005,OR=4.409)为影响成像质量的有意义指标.结论 BMI、腹水和腹壁静脉曲张为影响布加综合征患者肝脏MRE图像质量的主要因素.
Abstract
Objective To identify the main factors affecting the quality of liver magnetic resonance elastography(MRE)in patients with Budd-Chiari syndrome(BCS).Methods 59 patients with BCS diagnosed by DSA were selected.All patients un-derwent MRE.MRE image quality was independently analyzed by two radiologists and divided into high-and low-quality group.Gender,body weight,body mass index(BMI),liver fat content,liver iron content,ascites and abdominal collateral circulation-were included as factors.The consistency of MRE image quality evaluation by two doctors was analyzed by kappa test.Univariate analysis and logistic regression analysis were used to determine the decisive factors affecting liver MREin BCS patients.Re-sults The evaluation of MRE image quality was highly consistent among observers(kappa = 0.872).Univariate analysis showed that there were differences in BMI,liver fat fraction,liver iron deposition,ascites and abdominal varicose veins.Multivariate lo-gistic regression analysis showed that BMI(P = 0.015,OR = 1.888),ascites(P = 0.001,OR = 7.100)and abdominal varices(P = 0.005,OR = 4.409)were significant indexes affecting imaging quality.Conclusion BMI,ascites and abdominal varices are the main factors affecting the image quality of liver MRE in patients with BCS.
