摘要

目的 利用 320 排CT与半自动图像后处理插件,探讨冠状动脉周围脂肪组织(PCAT)与冠心病(CHD)的相关性.方法 选取冠状动脉CTA及冠状动脉造影(CAG)确诊的罪犯病灶位于左前降支(LAD)的CHD患者 52 例为观察组,并选取冠状动脉CTA结果未见明显病灶的患者 59 例为对照组.利用半自动图像后处理插件测量LAD开口处至远端 40 mm处的PCAT体积及密度,并对两组相关临床基线资料以及PCAT体积及密度进行比较;利用Pearson相关性分析PCAT体积与相关临床资料及超敏C反应蛋白(hs-CRP)的相关性;勾勒ROC曲线评估PCAT体积诊断CHD的最佳临界值;勾画箱形图及琴图描绘两组PCAT密度分布特征.结果 观察组PCAT体积高于对照组,差异具有统计学意义(P<0.05);ROC曲线显示,PCAT体积诊断CHD的曲线下面积(AUC)为 0.633,当PCAT体积为 1871 mm3 时,敏感度为73.1%,特异度为 52.5%,诊断CHD的效能最高;Pearson相关性分析显示PCAT体积与白细胞计数以及hs-CRP呈正相关(r=0.208、0.189,P<0.05);观察组PCAT的密度平均为(-88.35±5.08)HU,对照组平均密度为(-86.41±5.06)HU,观察组相比对照组较低,两组密度差异有统计学意义(P<0.05),并且箱形图及琴图描述了两组密度数据的离散及分布特征.结论 PCAT与CHD的发生发展有着一定的联系.

Abstract

Objective To investigate the correlation between pericoronary adipose tissue(PCAT)and coronary heart disease(CHD)using 320-slice CT and semi-automatic image post-processing plug-in.Methods A total of 52 CHD patients with cul-prit lesions located in the left anterior descending artery(LAD)confirmed by coronary CTA and coronary angiography(CAG)were selected as the experimental group,and 59 patients with no obvious lesions were selected as the control group.The semi-automatic image post-processing plug-in was used to measure the PCAT volume and density from the LAD opening to the distal 40mm,and the related clinical baseline data and PCAT volume and density were compared between the two groups.Pearson cor-relation was used to analyze the correlation between PCAT volume and clinical data and hS-CRP.ROC curve was delineated to evaluate the optimal critical value of PCAT volume for diagnosis of CHD.Delineated box diagram and harp diagram were employed to depict the density distribution characteristics of the two groups of PCAT.Results The volume of PCAT in the experimental group was higher than that in the control group,the difference was statistically significant(P<0.05);ROC curve showed that the area under the curve(AUC)of PCAT volume in the diagnosis of CHD was 0.633.When the PCAT volume was 1871mm3,the sen-sitivity was 73.1%and the specificity was 52.5%,which showed the highest efficiency in the diagnosis of CHD;Pearson correla-tion analysis showed that PCAT volume was positively correlated with leukocyte count and hs CRP(r = 0.208,0.189,P<0.05);the average density of PCAT in the experimental group was(-88.35±5.08)Hu,and the average density of LAD in the control group was(-86.41±5.06)Hu.Compared with the control group,the density difference between the two groups was statistically significant(P<0.05),and the box diagram and Piano diagram described the dispersion and distribution characteristics of the density data of the two groups.Conclusion PCAT is related to the occurrence and development of CHD.
