摘要
目的 探讨体素内不相干运动(intravoxel incoherent motion,IVIM)及扩散加权成像(diffusion weighted imag-ing,DWI)针对胃印戒细胞癌与非印戒细胞癌的诊断价值.方法 选取 70 例胃癌患者接受术前DWI及IVIM扫描,测量的参数包括表观扩散系数(apparent diffusion coefficient,ADC)、真扩散系数(pure diffusion coefficient,D)、灌注分数(perfu-sion fraction,f)及伪扩散系数(pseudodiffusion coefficient,D*).根据组织学分型将其分为印戒细胞癌(25 例)与非印戒细胞癌(45 例)两组.采用独立样本 t 检验比较上述参数的差异,利用ROC曲线分析其鉴别区分印戒细胞癌与非印戒细胞癌的诊断效率.结果 印戒细胞癌的ADC及D值均显著高于非印戒细胞癌,差异有统计学意义(P<0.05),而印戒细胞癌与非印戒细胞癌的D*及f值的差异无统计学意义(P>0.05).针对ADC及D值的ROC曲线分析表示,在区分印戒细胞癌与非印戒细胞癌上,D值与ADC值具有较高的诊断效能;D值与ADC的AUC值的差异不具有统计学意义(P>0.05).结论 IVIM及DWI可以较好的鉴别诊断印戒细胞癌与非印戒细胞癌,这对于胃印戒细胞癌的治疗方案的制定具有较大的意义.
Abstract
Objective To investigate the differential diagnostic value of distinguishing signet ring cell carcinoma(SRCC)from non signet ring cell carcinoma(NSRCC)by intravoxel incoherent motion(IVIM)and diffusion weighted imaging(DWI).Methods 70 patients with gastric adenocarcinoma were selected from Shandong Provincial Hospital.The patients underwent preoperative DWI and IVIM scans to measure IVIM and DWI parameters,including apparent diffusion coefficient(ADC),true diffusion coefficient(D),perfusion fraction(f),and pseudo diffusion coefficient(D*).According to histological type,they were divided into signet ring cell carcinoma(25 cases)and non signet ring cell carcinoma(45 cases).The differences were compared by independent sample t-tes tand the diagnostic efficiency of distinguishing SRCC from NSRCC was analyzed by ROC curves.Results ADC and D of SRCC were significantly higher than those of NSRCC(P<0.05),but there was no significant difference inD* and f between SRCC and NSRCC(P>0.05).With ROC curve analysis representation for ADC and D,in terms of distin-guishing SRCC from NSRCC,D and ADC values had high diagnostic efficiency;the difference of AUC between D and ADC was not statistically significant(P>0.05).Conclusion IVIM and DWI can better distinguish SRCC from NSRCC,which is of great significance for the formulation of treatment plan of SRCC.
