摘要
目的 利用MSCTA和CDFI观察肝总动脉起源变异的发生率及影像表现,为临床腹部疾病的诊治提供参考依据.方法 选取 287 例患者腹部MSCTA及部分CDFI影像资料,观察肝总动脉起源情况,记录相关数据并分析.结果 符合纳入标准的 230 例患者中肝总动脉起源变异共计 15 例,占总体的 6.50％.肝总动脉变异分为六种类型:Ⅰ型(肝总动脉起自肠系膜上动脉)9 例;Ⅱ型(肝总动脉起自腹主动脉)2 例;Ⅲ型(肝总动脉、胃左动脉和脾动脉均起自腹主动脉)1 例;Ⅳ型(腹腔干与肠系膜上动脉共干)1 例;Ⅴ型(肝总动脉和脾动脉共干起自肠系膜上动脉,)1 例;Ⅵ型(肝总动脉缺失)1 例.结论 肝总动脉变异影像学表现具有特征性,MSCTA结合超声可多角度反映血管变异情况,对临床诊断有重要指导意义.
Abstract
Objective MSCTA and CDFI were used to observe the incidence and imaging performance of variants in the ori-gin of the common hepatic artery to provide a clinical reference for the diagnosis and management of upper abdominal disease.Methods Retrospective analysis of 287 abdominal MSCTA and some CDFI images was performed to observe the origin of the common hepatic artery,and relevant data were recorded and analyzed.Results Among the 230 cases that met the inclusion cri-teria,a total of 15 cases had variants in the origin of the common hepatic artery,accounting for 6.50％of the total.There were six types of common hepatic artery variants:type Ⅰ(common hepatic artery from the superior mesenteric artery)in 9 cases;type Ⅱ(common hepatic artery from the abdominal aorta)in 2 cases;type Ⅲ(common hepatic artery,left gastric artery,and splenic artery from the abdominal aorta)in 1 case;type Ⅳ(celiac axis and superior mesenteric artery coaptation)in 1 case;type Ⅴ(common hepatic artery and splenic artery with common trunk from superior mesenteric artery)in 1 case;and Type Ⅵ(absence of common hepatic artery)found in 1 case.Conclusion The imaging manifestation of common hepatic artery variation is char-acteristic,and MSCTA combined with ultrasound can reflect the vascular variation in many aspects,which is an important guide for clinical diagnosis.
