摘要
目的 分析卵巢-附件报告及数据系统磁共振成像(O-RADS MRI)在附件肿瘤良、恶性诊断中的应用价值.方法 选取我院收治的 130 例附件肿瘤患者的临床及MRI检查资料,根据O-RADS MRI风险分层标准对每个肿块进行恶性风险评估,以病理组织活检结果为"金标准",绘制受试者工作特征(ROC)曲线,计算O-RADS MRI评分的诊断效能.结果 130 例附件肿瘤,病理组织活检显示恶性 36 例,良性 94 例;O-RADS 3、4、5 分肿块分别为 28、18、20 例,恶性率分别为 10.7%、83.3%、90.0%,O-RADS 1、2 分肿块分别为 6 例、58 例,病理均为良性;ROC曲线下面积为 0.964,95%CI 0.933～0.994;O-RADS MRI评分对附件肿瘤诊断的灵敏度、特异度、阳性预测值、阴性预测值及准确度分别为 91.7%、94.7%、86.8%、96.7%、93.9%.结论 O-RADS MRI评分对附件肿瘤良、恶性诊断具有较高价值,对临床诊疗方案的选择有积极作用.
Abstract
Objective To analyze the application value of Ovarian-Adnexal Reporting and Data System Magnetic Resonance Imaging(O-RADS MRI)in the diagnosis of benign and malignant adnexal tumors.Methods We conducted a retrospective analy-sis of the clinical and MRI data of 130 patients with adnexal tumors who were diagnosed and treated in our hospital,assessed ma-lignancy risk for each mass according to O-RADS risk stratification criteria.Using biopsy results as the"gold standard",we plot-ted the receiver operating characteristic(ROC)curve and calculated the diagnostic performance of magnetic resonance O-RADS.Results Among 130 cases of adnexal tumors,36 cases were malignant and 94 cases were benign in pathological bi-opsy;O-RADS 3,4,5 points were 28,18,and 20,respectively,and the malignant rates were 10.7%,83.3%,and 90.0%,re-spectively;O-RADS 1,2points were 6,58,and the pathology was benign;The area under the ROC curve was 0.964,95%CI:0.933 to 0.994;The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of O-RADS MRI score for the diagnosis of adnexal tumors were 91.7%,94.7%,86.8%,96.7%,and 93.9%,respectively.Conclusions O-RADS MRI score has high value in the diagnosis of benign and malignant adnexal tumors,which has a positive effect on the se-lection of clinical diagnosis and treatment programs.
