四维超声技术在孕中期胎儿畸形筛查中的临床应用

摘要
目的 探讨如何有效运用四维超声技术诊断胎儿畸形.方法 对 4225 例孕妇进行孕中期系统筛查,按照 28~32 个标准切面规范留图,并对所有资料进行回顾性分析.结果 4225 例孕妇筛查中,62 例胎儿发育异常共 68 处畸形,超声检查检出 60 例,发生率 1.47%,检出率 1.42%,畸形胎儿经超声心动图检查、引产及产后随访证实.检出畸形包括头颈面部畸形 15 例、胸腹部畸形 14 例、躯干四肢畸形 8 例及心脏血管畸形 25 例,其中心脏血管畸形构成比为40.32%,漏诊 1 例耳廓畸形和 1 例拇指寄生指.45 例实施羊膜腔内引产术,5 例采用水囊引产,7 例正常分娩,5 例孕晚期胎死腹中.结论 四维超声技术以高帧频、快速实时取样为特点,可以追踪胎儿运动,对胎儿发育可疑部位进行连续动态的立体成像,并可在对立体成像动态回放过程中进行补漏筛查.
Abstract
Objective To explore how to diagnose fetal malformation with four-dimensional ultrasonography.Methods 4225 pregnant women were screened systematically during middle pregnancy,and all data were retrospectively analyzed accord-ing to 28-32 standard sections.Results 4225 cases of pregnant women were screened,in which 62 cases were abnormal fetal development,68 were malformations,with incidence of 1.47%and detection rate of 1.42%,2 cases were missed diagnosis.The malformations were confirmed by echocardiography,the induced labor and postpartum follow-up.The detected malformations in-cluded 15 cases of head,neck and face malformations,14 cases of chest and abdomen malformations,8 cases of trunk and limbs malformations and 25 cases of cardiac vascular malformations,among which the constituent ratio of cardiac vascular malforma-tions was 40.32%,with 1 case of auricle malformations and 1 case of thumb thumb.Intramniotic induction of labor was per-formed in 45 cases,5 cases were induced with water bursa,7 cases were delivered normally,and 5 cases were stillborn in late pregnancy.Conclusion Four-dimensional ultrasound technology is characterized by high frame rate and fast real-time sam-pling.It can track fetal movement,perform continuous and dynamic stereo imaging of suspicious parts of fetal development,and perform leak trapping screening during dynamic playback of stereo imaging.
