摘要

目的 探讨鼻骨MSCT斜冠状位图像重建方法及诊断价值.方法 选取 176 例因外伤行鼻骨MSCT检查患者,并将所有患者图像进行两种不同角度冠状位重建(角度 1 为常规冠状位;角度 2 为斜冠状位).比较两种不同冠状位图像鼻骨骨折检出率及诊断结果准确率.结果 常规冠状位骨折检出率 84.93%(22 例假阴性),斜冠状位骨折检出率100%(3 例假阳性),组间比较差异有统计学意义(P=0.002).常规冠状位诊断单纯性鼻骨骨折、鼻骨复合骨折、单纯鼻中隔骨折及单纯上颌骨额突骨折分别是 76 例、36 例、4 例、8 例,诊断准确率为 54.00%;斜冠状位分别是 80 例、57 例、2 例、10 例,诊断准确率高达 96.00%,组间比较差异有统计学意义(P<0.001).结论 斜冠状位骨折检出率及诊断准确率明显高于常规冠状位,在临床应用中更具有优势.

Abstract

Objective To explore the method and diagnostic value of oblique coronal MPR of nasal bone with MSCT.Meth-ods 176 patients who underwent MSCT of nasal bone due to trauma were selected,retrospectively.Then,two different angle coronal reconstructions(angle 1 was routine coronal reconstruction;angle 2 was oblique coronal reconstruction)were conducted on all images.The detection rate and diagnostic accuracy of nasal bone fracture were compared.Results The detection rate of conventional coronal fractures was 84.93%(22 cases in false negative),and the detection rate of oblique coronal fractures was 100%(3 cases in false positive).The difference between groups was statistically significant(P=0.002).There were 76 cases of simple nasal bone fracture,36 cases of compound nasal bone fracture,4 cases of simple nasal septum fracture and 8 cases of simple maxillary frontal process fracture on routine coronal images,with the diagnostic accuracy of 54.00%;while,via oblique coronal reconstruction,those were 80 cases,57 cases,2 cases and 10 cases,correspondingly,with an accuracy of 96.00%.The difference between the groups was statistically significant(P<0.001).Conclusion The detection rate and diagnostic ac-curacy of oblique coronal reconstruction are significantly higher than that of conventional coronal,which has more advantages in clinical application.
