摘要
目的 探讨多模态超声检查对乳腺包裹性乳头状癌(EPC)不同病理亚型的诊断价值.方法 选取经病理诊断为乳腺EPC的 22 例患者,对比分析其三种亚型(单纯型乳腺EPC、乳腺EPC伴导管原位癌或乳腺EPC伴浸润性癌)的二维超声、彩色多普勒、三维超声、弹性成像及超声检查评分.结果 乳腺EPC大多呈囊实性回声,血流信号丰富,以Adler Ⅲ级多见,冠状面汇聚征少见;单纯型乳腺EPC、乳腺EPC伴导管原位癌或浸润性癌两组间差异无统计学意义(P>0.05);单纯型乳腺EPC多数形态规则、边缘光整;乳腺EPC伴导管原位癌或浸润性癌多数形态不规则、边缘不光整,乳腺EPC伴导管原位癌或浸润性癌弹性成像评分 5 分较多,且大多超声检查评分≥10 分,两组间差异有统计学意义(P<0.05).结论 应用多模态超声检查有助于提高对乳腺EPC不同病理亚型的诊断,为后续的精准治疗提供方向.
Abstract
Objective To investigate the diagnostic value of multimodal ultrasound in different pathological subtypes of en-capsulated papillary carcinoma(EPC).Methods The data of 22 patients with EPC diagnosed by pathology were selected,and the two-dimensional ultrasound,color Doppler,three-dimensional ultrasound,elastography and ultrasound scores of simple EPC and EPC with ductal carcinoma in situ or invasive carcinoma were compared and analyzed.Results EPCs showed cystic and solid echogenicity as well as rich blood flow signals with Adler grade Ⅲ being the common grade,whereas coronal convergence sign was less observed.There was no significant difference between the simple EPCs and EPCs with ductal carcinoma in situ or in-vasive carcinoma(P>0.05).Most of the simple EPCs had regular morphology and smooth margins,while most of the EPCs with ductal carcinoma in situ or invasive carcinoma had irregular morphology and margins.More EPCs with ductal carcinoma in situ or invasive carcinoma had elastic score of 5 and the majority of their ultrasound scores were≥10,and there was significant differ-ence between the two groups(P<0.05).Conclusion The application of multimodal ultrasound improves the diagnosis of di-verse pathological subtypes of EPC and contributes to the precision medicine for EPC.
