剪切波弹性成像在门静脉高压症TIPS疗效监测中的应用价值分析

摘要
目的 探讨剪切波弹性成像在经颈静脉肝内门体分流术(TIPS)疗效监测中的应用价值.方法 选取在四川省人民医院行TIPS术治疗的 43 例患者的临床资料,每例在术前、术后均行门静脉、脾静脉、肠系膜上静脉内径及血流速度测量,同时测量脾脏厚度,以及肝脏、脾脏剪切波速度(shear wave velocity,SWV)值,以门静脉压力梯度为金标准,观察术前、术后各指标的变化情况.结果 脾静脉、肠系膜上静脉内径以及肝脏剪切波速度术前、术后比较,差异无统计学意义(P>0.05),门静脉内径术后较术前差异有统计学意义(P<0.05),而TIPS术后门静脉、脾静脉、肠系膜上静脉血流速度明显增加,差异有统计学意义(P<0.05),门静脉压力梯度、脾脏SWV、脾脏厚度较术前减小,差异有统计学意义(P<0.05).结论 门静脉、脾静脉、肠系膜上静脉血流速度,脾脏厚度,脾脏SWV可作为TIPS术后疗效监测的重要指标.
Abstract
Objective To explore the application value of shear wave elastography in monitoring the efficacy of transjugular intrahepatic portosystemic shunt(TIPS).Methods The clinical data of 43 patients who underwent TIPS surgery at Sichuan Provincial People's Hospital were analyzed in this work.Before and after the operation,the diameter and blood flow velocity of portal vein,splenic vein,superior mesenteric vein were measured.At the same time,the thickness of spleen,as well as the shear wave velocity of liver and spleen were measured.With the portal vein pressure gradient as the gold standard,the changes of various indicators before and after the operation were analyzed and compared.Results The diameter of splenic vein,superior mesenteric vein and liver hear wave velocity had no significant change before and after operation(P>0.05).The difference in portal vein diameter was very small,whereas,the blood flow velocity of portal vein,splenic vein and superior mesenteric vein in-creased after TIPS(P<0.05).The portal vein pressure gradient,the thickness of spleen shear wave velocity,and spleen de-creased compared with that before operation(P<0.05).Conclusion The blood flow velocity of portal vein,splenic vein,supe-rior mesenteric vein,spleen thickness,and spleen shear wave velocity can be used as important indicators for monitoring the effi-cacy of TIPS.
