摘要
目的 探讨CalliSpheres可载药微球动脉化疗栓塞治疗膀胱癌并出血的可行性及安全性.方法 选取接受CalliSpheres可载药微球经导管动脉化疗栓塞(DEB-TACE)治疗的 11 例膀胱癌并出血患者,采用改良实体瘤疗效评价标准(mRECIST)评价 3 个月、6 个月的治疗效果,并记录患者治疗前后血尿复发情况、血液指标变化、不良反应及并发症发生情况.结果 11 例患者均完成介入栓塞治疗,技术成功率 100%.肉眼血尿均在术后 48～72 h消失.患者首次治疗后3 个月和 6 个月的客观溶解率(ORR)分别为 72.73%和 63.64%,疾病控制率DCR分别为 90.91%和 81.82%.随访 9个月时 11 例患者均生存.术后并发症包括膀胱区疼痛 8 例,恶心、呕吐 6 例,发热 2 例,均经对症处理后缓解.未出现异位栓塞等严重并发症.结论 应用CalliSpheres载药微球动脉栓塞化疗治疗膀胱癌并出血疗效显著,安全性好,值得推广.
Abstract
Objective To investigate the feasibility and safety of CalliSpheres drug-loaded microsphere arterial chemoemboli-zation in the treatment of bladder cancer complicated with bleeding.Methods Eleven patients with bladder cancer and Bleed-ing who received CalliSpheres drug-carrying embolization microspheres transcatheter arterial chemoembolization from January 2019 to March 2022 were selected and evaluated by Modified solid Tumor Efficacy Evaluation Criteria(mRECIST)for 3 and 6 months.The recurrence of hematuria,changes in blood indexes,adverse reactions and complications were recorded before and after treatment.Results All the 11 cases were treated with interventional embolization with a success rate of 100%.Gross he-maturia disappeared within 48-72h after surgery.In this study,the ORR of 3 and 6 months after the first treatment was 72.73%and 63.64%,and the DCR was 90.91%and 81.82%,respectively.At 9-month follow-up,all 11 patients survived.Postoperative complications included bladder pain in 8 cases,nausea and vomiting in 6 cases and fever in 2 cases,all of whom were relieved after symptomatic management.There were no ectopic embolism and other serious complications.Conclusion CalliSpheres drug-loaded microsphere arterial embolization is effective and safe in the treatment of bladder cancer complicated with bleeding.
