摘要

目的 采用超声弹性参数动态评估超声引导下激素联合针刀治疗足底筋膜炎(PF)前、后足底筋膜弹性的变化,并结合临床视觉模拟量表(VAS)、足踝功能评分表(AOFAS)分析,探讨治疗的有效性及超声弹性参数在足底筋膜炎疗效评估中的应用价值.方法 选取 34 例确诊为PF并进行超声引导下激素联合针刀治疗的患者,分别于治疗前、治疗后 7 天、1 个月、3 个月测量足底筋膜厚度、弹性图像评分、弹性应变比,同时使用VAS、AOFAS评分表进行评估,分析各参数治疗前、后的动态变化及相关性.结果 治疗后 1 个月弹性图像评分显示足底筋膜硬度增加(P<0.05),治疗后 3 个月弹性图像评分显示足底筋膜硬度与治疗前相比差异有显著统计学意义(P<0.01).治疗后 7 天、治疗后 1 个月弹性应变比与治疗前相比差异无统计学意义(P>0.05),治疗后 3 个月弹性应变比显著增加(P<0.01).治疗 7 天后VAS评分显示治疗后分值下降(P<0.01),而AOFAS评分增加(P<0.01).相关分析显示弹性图像评分与VAS评分、AOFAS的评分的相关性(相关系数r分别为 0.695、-0.668)高于足底筋膜厚度、弹性应变比与临床评分的相关性(相关系数r分别为:0.535、-0.532 及-0.482,0.443).结论 超声弹性参数能反映足底筋膜炎治疗前、后的动态变化,可以有效评估治疗效果;超声引导下激素联合针刀治疗PF初期虽临床评分有好转,但足底筋膜弹性的恢复需要一定时间.

Abstract

Objective This study aimed to dynamically evaluate the changes of plantar fascia elasticity before and after the treatment of plantar fasciitis(PF)with hormone combined with needle knife under the guidance of ultrasound by using ultrasonic elasticity parameters,and to explore the effectiveness of treatment and the application value of ultrasonic elasticity parameters in the evaluation of curative effect of plantar fasciitis by combining the analysis of visual analog scale(VAS)and ankle function score scale(AOFAS).Methods 34 patients who were diagnosed as PF and treated with ultrasound-guided hormone combined with needle knife therapy were measured the plantar fascia thickness,ultrasound elastography analysis elastic image score and elastic strain ratio before treatment,7 days,1 month,and 3 months after treatment.At the same time,VAS and AOFAS scoring tables were used to evaluate the dynamic changes and correlations of the parameters before and after treatment.Results One month after treatment,the elastic image score showed that the hardness of plantar fascia increased(P<0.05).Three months after treatment,the elastic image score showed that the hardness of plantar fascia had significant difference compared with that before treatment(P<0.01).The elastic strain ratio at 7 days after treatment and 1 month after treatment had no significant difference compared with that before treatment(P>0.05),but it increased significantly at 3 months after treatment(P<0.01).VAS score decreased after 7 days of treatment,and AOFAS score increased(P<0.01).Correlation analysis showed that the correlation be-tween elastic image score and VAS score,AOFAS score(correlation coefficient r was 0.695,-0.668 respectively)was higher than that between plantar fascia thickness,elastic strain ratio and clinical score(correlation coefficient r was 0.535,-0.532 and-0.482,0.443 respectively).Conclusion Ultrasound elastic parameters can reflect the dynamic changes of plantar fasciitis be-fore and after treatment,and can effectively evaluate the treatment effect.Although the clinical score of hormone combined with acupotomy under ultrasound guidance is improved in the early stage of PF treatment,the recovery of plantar fascia elasticity takes some time.
