摘要

目的 探讨 3D打印技术在前交通动脉瘤栓塞治疗中的应用价值.方法 选取就诊于我院经脑动脉造影确诊前交通动脉瘤患者 40 例,随机分为两组,观察组与对照组各 20 例.其中对照组术者术中正常造影后凭借手术经验直接塑形微导管.观察组则通过处理后的 3D-DSA造影数据,制作前交通动脉瘤模型,在术前辅助微导管塑形.比较两组微导管首次塑形成功率、微导管末次塑形后进入指引导管至到达理想释放弹簧圈位置的时间、术后即时评分情况、术后至出院的恢复时间、术后 6 个月随访阴性率.结果 观察组患者术中微导管首次塑形成功率 75%(15/20),微导管末次塑形后通过指引导管至到达理想释放弹簧圈位置时间(49.35±9.49)s,术后即时评分 4 分(4,4),术后至出院的恢复时间(3.25±1.02)天.对照组患者术中微导管首次塑形成功率 40%(8/20),微导管末次塑形后通过指引导管至到达理想释放弹簧圈位置时间(79.85±17.47)s,术后即时评分 3 分(3,4),术后至出院的恢复时间(4.15±1.27)天.两组比较,差异有统计学意义(P<0.05).而观察组患者术后 6 个月随访阴性率为 95%,对照组患者术后 6 个月随访阴性率为 90%,差异无统计学意义(P>0.05).结论 相较于传统微导管塑形方式,3D打印技术的应用,可使术中微导管首次塑形成功率提高、缩短手术操作时间及术后恢复时间、显著提高塑形微导管的精确度与稳定性及治疗效果.

Abstract

Objective To explore the application value of 3D printing technology in the treatment of anterior traffic artery em-bolization.Methods Forty patients diagnosed with anterior communicating artery aneurysm through cerebral angiography in our hospital were randomly divided into two groups,with 20 cases in the observation group and 20 cases in the control group.In the control group,the operators directly shaped the microcatheter based on their surgical experience after normal angiography during the surgery.In the observation group,a 3D-DSA angiography model of the anterior communicating artery aneurysm was created using processed data,and the microcatheter was shaped with the assistance of the model before the surgery.The success rate of initial microcatheter shaping,time from the last shaping of microcatheter to reaching the desired position for releasing the spring coil,immediate postoperative scores,recovery time from postoperative to discharge,and negative rate of follow-up after 6 months were compared between the two groups.Results The success rate of the first shaping of the microcatheter during sur-gery in the observation group was 75%(15/20).After the last shaping of the microcatheter,the time to reach the ideal release spring position through the guiding catheter was(49.35±9.49)seconds.Immediate postoperative score was 4(4,4)points,and the recovery time from postoperative to discharge was(3.25±1.02)days.In the control group,the success rate of the first shaping of the microcatheter during surgery was 40%(8/20).After the last shaping of the microcatheter,the time to reach the ideal release spring position through the guiding catheter was(79.85±17.47)seconds.Immediate postoperative score was 3(3,4)points,and the recovery time from postoperative to discharge was(4.15±1.27)days.The differences between the two groups were statistically significant(P<0.05).However,the negative rate of follow-up in the experimental group was 95%after 6 months,while it was 90%in the control group,and the difference was not statistically significant(P>0.05).Conclusion Com-pared with the traditional microcatheter shaping method,the application of 3D printing technology can improve the success rate of microcatheter shaping in surgery for the first time,reduce the operation time and postoperative recovery time,significantly im-prove the accuracy,stability,and surgical outcome of shaped microcatheters.
