CT引导穿刺引流术联合依达拉奉治疗高血压脑出血的临床观察

摘要
目的 探讨CT引导下血肿穿刺联合依达拉奉治疗高血压脑出血(HICH)的临床价值.方法 选取我院接受治疗的HICH患者 82 例,根据随机数字表法分为治疗组 41 例,对照组 41 例.所有受试者均接受CT引导下血肿穿刺引流术并予以常规对症和支持治疗,在此基础上治疗组加用依达拉奉治疗,两组均持续治疗 3 个疗程.记录整体治疗效果及期间不良反应发生情况,并分别于治疗前后测定两组Barthel指数(BI)评分、美国国立卫生院神经功能缺损(NIHSS)评分、炎症反应、氧化损伤指标水平、CT特征变化.结果 治疗后,治疗组的总有效率为 95.12%,高于对照组的 78.05%,组间差异有统计学意义(χ2=5.145,P=0.023);两组BI评分治疗组高于对照组,而两组IL-8、PTX3、VCAM1、MAD、IMA、ROS以及NIHSS评分治疗组低于对照组,差异均有统计学意义(P<0.05);治疗后,治疗组出血量、中线移位、血肿扩大例数均显著低于对照组,差异有统计学意义(P<0.05);另外,两组不良反应发生率比较差异无统计学意义(P>0.05).结论 CT引导下血肿穿刺引流术联合依达拉奉能够显著改善HICH患者炎症、氧化损伤状态,缓解临床症状.
Abstract
Objective To observe the clinical effect of CT-guided hematoma drainage combined with edaravone in treating hypertensive intracerebral hemorrhage(HICH).Methods A total of 82 patients with HICH treated in our hospital from March 2021 to March 2023 were selected and randomly divided into a control group(n=41)and a treatment group(n=41)according to a random number table method.All subjects underwent CT-guided hematoma drainage and received routine symptomatic and sup-portive treatment.In addition,the treatment group was treated with edaravone,and both groups received continuous treatment for 3 courses.The overall therapeutic effect and occurrence of adverse reactions during the treatment were recorded.The Barthel In-dex(BI)scores,National Institutes of Health Stroke Scale(NIHSS)scores,inflammation response,oxidative damage index lev-els,and changes in CT features of both groups were measured before and after treatment.Results After treatment,the total ef-fective rate in the treatment group was 95.12%,which was higher than that in the control group(78.05%),and the difference be-tween the two groups was statistically significant(χ2=5.145,P=0.023);both groups showed a significant increase in BI scores,with the treatment group higher than the control group.In addition,IL-8,PTX3,VCAM1,MAD,IMA,ROS,and NIHSS scores were significantly reduced in both groups,with the treatment group lower than the control group(P<0.05).After treat-ment,the bleeding volume,midline displacement,and number of cases of hematoma enlargement in the treatment group were significantly lower than those in the control group(P<0.05).Moreover,there was no statistically significant difference in the inci-dence of adverse reactions between the two groups(P>0.05).Conclusion CT-guided hematoma drainage combined with edara-vone can significantly improve the inflammatory and oxidative damage status of HICH patients,further alleviate clinical symp-toms,improve quality of life and neurological function,and achieve significant therapeutic effects.
