摘要

目的 探讨 18F-FDG PET/CT显像患者中偶然发现甲状腺摄取的临床意义.方法 选取在我科因怀疑各种非甲状腺病变而进行 18F-FDG PET/CT 检查的 2817 例患者的图像,其中 245 例(8.7%)甲状腺 18F-FDG摄取增加,分析其影像学表现,计算最大标准摄取值(SUVmax),结合患者的年龄、性别、进一步检查、检验、手术及随访结果进行综合分析.结果 在 245 例甲状腺 18F-FDG摄取增加患者中 84 例(34.3%)呈弥漫性摄取,161 例(65.7%)呈局灶性摄取.所有经进一步检查的弥漫性摄取患者均被诊断为良性疾病,而接受病理诊断的 49 例局灶性摄取的患者中 29 例(59.2%)为良性病变,20 例(40.8%)为恶性病变.恶性病变和良性病变患者的SUVmax值分别为 8.92±6.60 和 5.57±3.48,差异有统计学意义(P<0.05).结论 在 18F-FDG PET/CT检查中偶然发现甲状腺摄取的患者中,弥漫性摄取通常伴有良性甲状腺疾病,而局灶性摄取需要进行进一步的检查评估,更有利于提高准确率.

Abstract

Objective To investigate the clinical significance of incidental thyroid uptake in patients subjected to 18F-FDG PET/CT imaging.Methods Images of 2817 patients who underwent 18F-FDG PET/CT in our department for suspected various non-thyroidal lesions were selected,of whom 245(8.7%)had increased thyroid 18F-FDG uptake.Their imaging was analyzed,and standardized uptake values(SUVmax)were calculated,combined with the patients'age,gender,examination,surgery and follow-up results.Results 84(34.3%)of the 245 patients with thyroid 18F-FDG uptake increased showed diffuse uptake and 161(65.7%)showed focal uptake.All patients with diffuse uptake were diagnosed with benign disease,while 29(59.2%)of the 49 patients with focal uptake who underwent pathological diagnosis had benign lesions and 20(40.8%)had malignant lesions.The SUVmax were(8.92±6.60)and(5.57±3.48)for patients with malignant and benign lesions,with statistically significant differences(P<0.05).Conclusion In patients with incidental thyroid uptake on 18F-FDG PET/CT,diffuse uptake is usually associated with benign thyroid disease,whereas,focal uptake requires further examination and evaluation,which is more benefi-cial to improve the accuracy.
