示卓安超声造影在小肝癌微波消融术中的应用价值

摘要
目的 探讨示卓安超声造影在小肝癌微波消融术中的应用价值.方法 选取本院在超声引导下微波消融治疗的小肝癌患者 25 例.依据肿瘤造影前、动脉期、枯否期声像图显影清晰度进行评分,比较三个时期声像图质量.消融后 1 个月行增强影像检查以评估消融近期疗效.结果 肿瘤声像图显影清晰度评分:造影前(2.7±0.5)分、动脉期(4.8±0.4)分、枯否期(4.8±0.4)分.动脉期与枯否期肿瘤显影清晰度均优于造影前,评分差异均有统计学意义(P<0.05),动脉期与枯否期肿瘤显影清晰度相近,评分差异无统计学意义(P>0.05),但枯否期肿瘤显影时间特别长,有利于做精准的穿刺消融.消融后 1 个月增强影像显示所有患者均达到完全消融.结论 在示卓安超声造影的枯否期肝癌可以长时间清晰显影,降低了穿刺难度,有利于术者实施精准的布针消融,从而有可能取得更好的消融效果.
Abstract
Objective To explore the application value of Sonazoid in microwave ablation of small hepatocellular carcinoma.Methods 25 patients with small hepatocellular carcinoma were treated with microwave ablation guided by ultrasound.The sono-graphic quality of the three stages was compared based on the score of the sonographic clarity,i.e.,before CEUS,during arterial phase,and during kupffer phase.One month after ablation,the enhanced imaging was performed to evaluate the short-term effi-cacy of ablation.Results The sonographic clarity scores were(2.7±0.5)before CEUS,(4.8±0.4)during arterial phase,and(4.8±0.4)during kupffer phase.The clarity of tumors in arterial phase and kupffer phase was better than that before CEUS,and the score difference was statistically significant(P<0.05).The clarity of tumors in arterial phase and kupffer phase was similar,with no statistically significant difference in score(P>0.05).However,the clear display time of kupffer stage was long,which was conducive to accurate puncture and ablation.One month after ablation,the enhanced imaging showed that all patients had achieved complete ablation of their hepatocellular carcinoma.Conclusion During the kupffer phase of Sonazoid,hepatocellu-lar carcinoma can be clearly displayed for a long time,which reduces the difficulty of puncture and facilitates accurate ablation by surgeons.It is possible to achieve better ablation effects.
