输卵管卵巢脓肿特征性CT表现的分析

摘要
目的 探讨输卵管卵巢脓肿在MSCT检查中对诊断有价值的特征性表现.方法 选取经手术病理证实的输卵管卵巢脓肿患者 56 例,术前均行CT增强扫描检查,分析临床及影像学资料.结果 输卵管卵巢脓肿患者中单独右侧发病 28 例(50%),单独左侧发病 18 例(31.14%),双侧发病 10 例(17.85%).管道型输卵巢卵巢脓肿 26 例(46.43%),串珠型为 16 例(28.57%),多囊型为 12 例(21.42%),单囊型 2 例(3.57%).各种类型的输卵管卵巢脓肿灶的短径比较,差异有统计学意义(P=0.016),长径/短径的比较差异有统计学意义(P=0.024),线性囊灶出现于 60.71%的输卵管卵巢脓肿,在管道型与其他各分型中所占比例差异有统计学意义(P=0.014).输卵管卵巢脓肿的囊壁厚度介于 0.21～0.35 cm 之间,增强扫描厚壁及分隔可见中重度强化,静脉期达到峰值,平均强化程度为 43～51HU.结论 通过对输卵巢卵巢脓肿CT征象的观察,管道型结构和线性囊灶的出现可提示诊断输卵管卵巢脓肿,对提高影像科医师的准确诊断有一定帮助.
Abstract
Objective To analyze the multi-slice spiral CT imaging manifestations of tubo-ovarian abscess,and to summa-rize the imaging features that are valuable for diagnosis.Methods A retrospective analysis of 56 patients with tubo-ovarian ab-scess confirmed by surgery and pathology was performed.All of them underwent preoperative CT enhanced scan,and the clinical and imaging data were perfect.Results Among the patients with tubal and ovarian abscess,28 cases were found on the right side alone,accounting for 50%of the total number of patients,which was the most common compared with bilateral and left side alone(10 cases,18 cases).Pipeline type was the most common type of ovarian abscess in 26 cases(46.43%),with beaded type and polycystic type occurring in 16 cases(28.57%)and 12 cases(21.42%),with monocystic type in 2 cases(3.57%),respec-tively.There were statistically significant differences in the short diameter(P=0.016)and long diameter/short diameter(P=0.024)of various types of tubal ovarian abscess.Linear capsular lesions were found in 60.71%of tubal and ovarian abscesses,and were most common in duct type,the difference was statistically significant(P=0.014).The thickness of the cyst wall of the tubal ovarian abscess was between 0.21cm and 0.35cm,and moderate to severe enhancement could be seen in the thick wall and the separation on enhanced scan.The peak value was reached in the venous stage,and the average degree of enhancement was between 43HU to 51HU.Conclusion Through the observation of the signs of tubo-ovarian abscess,the appearance of duct-shaped structures and linear cyst foci can prompt the diagnosis of tubo-ovarian abscess,which is helpful to improve the diagnosis of radiologists.
