MRI与CT诊断膝关节损伤的临床应用分析

摘要
探讨术前用三维 CT 行胫骨平台骨折 Schatzker 分型后,应用 MRI 检查诊断膝关节损伤的临床价值.方法 选取胫骨平台骨折患者 124 例,术前应用三维CT检查扫描行胫骨平台骨折Schatzker分型,同时应用MRI评估膝关节半月板损伤和韧带损伤的程度.结果 MRI诊断膝半月板损伤 98 例,MRI检查准确率、特异度、灵敏度分别为86.29%、71.43%、89.32%.MRI显示半月板损伤程度,Ⅰ度 15 例,Ⅱ度 45 例、Ⅲ度 38 例.根据 CT胫骨平台骨折Schatzker 分型,Ⅰ 型、Ⅱ 型半月板损伤 Ⅱ 度百分比为 69.57%和 58.62%,Ⅴ 型和 Ⅵ 型半月板损伤 Ⅲ 度百分比为61.54%和 64.29%.结论 术前CT行胫骨平台骨折Schatzker分型,应用MRI可了解胫骨平台骨折合并膝半月板及韧带损伤.
Abstract
Objective To study Schatzker classification of tibial plateau fracture with three-dimensional CT before opera-tion,and to evaluate the value of meniscus and ligament injury in tibial plateau fracture by wse of MRI.Methods 124 cases of articular cartilage injury were selected and Schatzker classification of tibial plateau fractures was performed with three-dimensional CT before operation,MRI was used to evaluate the degree of meniscus injury and ligament injury.Results There were 98 of cases knee meniscus injury by MRI diagnosis.The accuracy,specificity and sensitivity of MRI were 86.29%,71.43%and 89.32%.MRI showed meniscus injury in 15,45 and 38 cases of degree I,degree Ⅱ and degree Ⅲ.The percentage of de-gree Ⅱ meniscus injury was 69.57%and 58.62%in type Ⅰ and type Ⅱ meniscus injury,and 61.54%and 64.29%in type Ⅴ and Type Ⅵ meniscus injury.Conclusion Based on Schatzker classification of tibial plateau fracture by preoperative CT,MRI can be used to understand the tibial plateau fracture with meniscus injury and ligament injury,and provide effective information for clinic.
