摘要
目的 调研CT/MRI对比剂的应用现状,了解存在的问题,为合理规范使用对比剂提出合理建议.方法 以调查问卷的方式选取山东省内 313 所二级以上医疗机构对比剂使用情况,问卷内容包括增强检查比率,对比剂使用权限以及不良反应率等问题.结果 三级甲等、三级乙等、二级医院的CT/MRI增强检查的强化率存在差异,随着医院等级的提升,强化率逐渐增加;强化检查申请主要有临床科室医师开具(78.27%),仅有少部分医院增强检查可以由放射科开具(19.49%);60.00%以上的医疗机构在临床开具申请单时需要注明对比剂名称,对比剂不良反应率在采集后没有变化.结论 影像检查强化率反映出了二、三级医疗机构间影像检查项目的差别.放射科作为对比剂使用科室,对其特性和适应证的掌握情况应高于其他科室,因此判定患者是否需要强化扫描以及使用的对比剂种类应由放射科主导,但从调研结果显示执行情况存在较大差异,需要提升规范化应用和依据.
Abstract
Objective To investigate the application of CT/MRI contrast agents,to explore the existing problems,and to put forward reasonable suggestions for the reasonable and standardized use of these contrast agents.Methods A survey of contrast agent application in 313 medical institutions in Shandong province was conducted by means of questionnaire,including en-hanced examination ratio,use authority of contrast agents,and adverse reaction rate.Results The enhancement rate of the CT/MRI was significant different in Grade Ⅲ A,Grade Ⅲ B and Grade Ⅱ hospitals.Highter grade hospital had higher enhancement rate;the application for enhanced examination was mainly issued by doctors in clinical departments(78.27％),and only in a small number of hospitals it could be issued by the radiology department(19.49％).More than 60％of the medical institutions needed to indicate the name of the contrast agents when issuing the clinical application form.The adverse reaction rate of contrast agents remained unchanged after the centralized procurement of drugs.Conclusion As a department of contrast use,the radiol-ogy department should know its characteristics and indications better than other departments.Therefore,whether the patient needs enhanced examinations and the type of contrast should be led by the radiology department.But the investigation results are quite different,and the standard and basis need to be proposed.
