多序列MRI对高血压性脑出血的诊断价值分析

摘要
目的 探讨多序列MRI对高血压性脑出血的诊断价值.方法 选取 134 例疑似高血压性脑出血患者,所有患者入院后均接受多序列磁共振成像(MRI)和头颅电子计算机断层扫描(CT)检查,以CT检查结果为依据,分析多序列MRI对高血压性脑出血的诊断效能及其诊断脑出血部位与头颅CT检查结果的一致性.结果 头颅CT检查结果显示,134 例疑似高血压性脑出血患者中确诊高血压性脑出血 114 例,其中基底节出血 50 例,丘脑出血 13 例,脑叶出血 35 例,小脑出血 10 例,脑室出血 6 例.以头颅CT检查结果为参照,多序列MRI诊断高血压性脑出血的准确度为 92.54%(124/134),灵敏度为 96.49%(110/114),特异度为 70.00%(14/20).多序列MRI诊断脑出血部位与头颅CT诊断结果的一致性较高(Kappa=0.787,P<0.001).结论 多序列MRI诊断高血压性脑出血具有良好的准确性,且其诊断脑出血部位与头颅CT诊断结果具有较高的一致性.
Abstract
Objective To explore the diagnostic value of multi sequence MRI in hypertensive intracerebral hemorrhage.Method 134 suspected hypertensive intracerebral hemorrhage patients admitted to the hospital were selected as the study sub-jects.All patients underwent multi sequence magnetic resonance imaging(MRI)and head computed tomography(CT)examina-tion after admission.Based on the results of CT examination,the diagnostic efficacy of multi sequence MRI for hypertensive intra-cerebral hemorrhage and the consistency between the diagnosis of intracerebral hemorrhage and the results of head CT examina-tion were analyzed.Results The results of head CT examination showed that 114 cases of hypertensive intracerebral hemor-rhage were diagnosed in 134 patients with suspected hypertensive intracerebral hemorrhage,including 50 cases of basal ganglia hemorrhage,13 cases of thalamic hemorrhage,35 cases of lobar hemorrhage,10 cases of cerebellar hemorrhage,and 6 cases of ventricular hemorrhage.Based on the results of head CT examination,the accuracy of multi sequence MRI in diagnosing hyper-tensive intracerebral hemorrhage was 92.54%(124/134),sensitivity was 96.49%(110/114),and specificity was 70.00%(14/20).The consistency between the multi sequence MRI diagnosis of the location of cerebral hemorrhage and the CT diagnosis of the head was high(Kappa=0.787,P<0.001).Conclusion Multi sequence MRI has good accuracy in diagnosing hypertensive intracerebral hemorrhage,and its diagnosis of the location of intracerebral hemorrhage is highly consistent with the results of skull CT diagnosis.
