颈部淋巴结结核超声检查表现及其预后分析

摘要
目的 探讨颈部淋巴结结核的超声特征,分析超声检查不同声像图表现与颈部淋巴结结核预后的关系.方法 选取在北京胸科医院经病理确诊并接受规律抗结核治疗的 109 例颈部淋巴结结核患者超声检查不同声像图表现,根据液化情况分为两组:无液化及液化范围<50%的淋巴结作为A组(63 例),液化范围≥50%的淋巴结作为B组(46 例),停药时比较两组淋巴结的缩小率,分析两组的预后是否存在差异.结果 规律抗结核至患者停药,两组病例中A组淋巴结缩小率大于B组,差异有统计学意义(P<0.001).结论 A组淋巴结抗结核治疗后预后更好,超声检查可以全程观察颈部淋巴结结核患者的淋巴结情况、对疾病的诊断及预后有指导价值.
Abstract
Objective To analyze the ultrasound characteristics of cervical lymph node tuberculosis and to explore the rela-tionship between different ultrasound imaging manifestations and the prognosis of cervical lymph node tuberculosis.Methods Different ultrasonographic manifestations of 109 patients with cervical lymph node tuberculosis diagnosed pathologically and re-ceiving regular anti-tuberculosis therapy in Beijing Chest Hospital from January 2018 to October 2022 were collected.They were divided into two groups based on liquefaction status.Lymph nodes without liquefaction and with a liquefaction range of less than 50%were classified as Group A,with a total of 63 cases.The lymph nodesof other groupwere classified as Group B,with a total of 46 cases.The reduction rate of lymph nodes in the two groups was compared when stopping the medication.We analyzed whether there was a difference in prognosis between the two groups.Results Regular anti-tuberculosis therapy was used until the patient stopped taking medication.In the two groups of cases,the reduction rate of partially liquefied lymph nodes was higher than that of the other groups,with a statistically significant difference(P=0.00).Conclusion GroupA has a better prognosis af-ter anti-tuberculosis treatment.Ultrasonic testing can observe the lymph node condition of cervical lymph node tuberculosis pa-tients throughout the process,thereby providing guidance for the diagnosis and prognosis of patients.
