超声检查多模态评估RSA子宫畸形患者ER的应用价值

摘要
目的 探讨复发性自然流产(RSA)子宫畸形患者发生子宫内膜容受性(ER)采用超声检查多模态的应用价值.方法 选取在我院诊治的子宫畸形RSA患者 150 例作为观察组,再选取同期单纯子宫畸形患者 150 例作为对照组,比较两组超声检查多模态参数的分布情况,并分析超声检查多模态对子宫畸形患者RSA的诊断价值.结果 观察组子宫内膜厚度(ET)、内膜类型、内膜运动、内膜及内膜下血流分布、内膜容积(EV)、内膜血管血流指数(EVFI)分布与对照组比较,差异均有统计学意义(P<0.05).观察组超声检查多模态评分为 9.92±2.10,明显低于对照组的 14.80±1.62,差异有统计学意义(P<0.05).超声检查多模态各参数ET、内膜类型、内膜运动、内膜及内膜下血流分布、EV、内膜血管血流指数均呈正相关(P<0.05).ROC曲线分析显示,超声检查多模态评分诊断子宫畸形患者RSA的AUC为 0.963,诊断的敏感度为 85.30%,特异度为 94.00%.结论 与单纯子宫畸形患者相比,子宫畸形患者RSA的超声检查多模态评分降低,超声检查多模态评分可评估子宫畸形患者RSA的ER,且对子宫畸形患者RSA具有较高的诊断价值.
Abstract
Objective To explore the application value of multimodal ultrasound examination in the development of endome-trial receptivity(ER)in patients with recurrent spontaneous abortion(RSA)uterine malformations.Methods 150 RSA pa-tients with uterine malformations treated in our hospital were selected as the observation group,and 150 patients with simple uter-ine malformation during the same period were selected as the control group.The distribution of ultrasound multimodal parameters between the two groups was compared,and the diagnostic value of ultrasound multimodality for RSA in uterine malformation pa-tients was analyzed.Results The observers group showed statistically significant differences in the endometrial thickness(ET),the endometrial type,the endometrial movement,the distribution of endometrial and the subendometrial blood flow,the endometrial volume(EV),and distribution of endometrial vascular flow index(EVFI)compared to the control group(P<0.05).The ultrasonic multi-mode score of the observers group was(9.92±2.10),which was significantly lower than that of the control group(14.80±1.62)(P<0.05).The parameters of ultrasound multimodality,such as ET,endometrial type,endometrial move-ment,endometrial and subendometrial blood flow distribution,EV,and endometrial vascular blood flow index,were positively correlated(P<0.05).The analysis of the ROC curve of the subjects showed that the AUC of the ultrasonic multi-mode score in di-agnosing RSA in patients with uterine malformations was 0.963,the sensitivity of diagnosis was 85.30%,and the specificity of di-agnosis was 94.00%.Conclusion Compared with patients with simple uterine malformation,the ultrasonic multimodal score of RSA in patients with uterine malformation is lower.The ultrasonic multimodal score can evaluate the ER of RSA in patients with uterine malformation,and has higher diagnostic value for RSA in patients with uterine malformation.
