应用MRI T2mapping成像量化急性前交叉韧带损伤患者患侧与对侧胫股关节软骨的分析

摘要
目的 探讨应用T2mapping量化比较急性前交叉韧带(ACL)损伤患者患侧与正常侧关节软骨基质的变化.方法 选取急性及亚急性ACL损伤患者(创伤后平均 32 天)30 例,患者均进行双侧膝关节MRI检查,包括常规MRI检查及T2mapping序列,之后对胫股关节不同部位深浅层关节软骨进行测量T2 值及关节软骨厚度,运用配对t检验评估患侧及正常关节软骨的差异.结果 与正常膝关节相比,患侧外侧缘浅表股骨中央前部、股骨后缘后部,外侧缘深层股骨后缘后部,内侧缘浅表股骨前缘后部,内侧缘深层股骨中央后部、胫骨前部、胫骨中央、胫骨后部等部位患侧关节软骨T2值明显较高,差异具有统计学意义(P<0.05).患侧内外侧缘深浅层关节软骨T2 值相比,除了外侧缘股骨后缘后部及内侧缘股骨前缘后部外,其余所有部位浅层T2 值较深层明显较高,差异具有统计学意义(P<0.05).外侧缘浅表部位股骨前缘前部及内侧缘深层部位胫骨前部,患侧关节软骨T2 值较正常侧低,差异具有统计学意义(P<0.05).内侧缘股骨前缘后部、胫骨后部等部位患侧关节软骨厚度较正常侧薄,差异具有统计学意义(P<0.05).结论 ACL损伤患者患侧关节软骨多处T2 弛豫时间较对侧升高并且多处关节软骨变薄.ACL损伤是PTOA发生发展的重要原因.
Abstract
Objective To conduct a quantitative comparison of changes in the articular cartilage matrix between the affected and normal sides in patients with acute ACL injury using T2 mapping.Methods Thirty patients with acute and subacute ACL in-juries(average 32 days after trauma)were collected,and all patients underwent bilateral knee MRI examinations,including con-ventional MRI examinations and T2mapping sequences,followed by measurement of deep and superficial articular cartilage in dif-ferent parts of the tibiofemoral joint.T2 values and articular cartilage thickness were measured,and the differences between the affected and normal articular cartilage were evaluated using paired t-tests.Results Compared with the normal side,T2 values were higher in aCF,pPFlateral superficial zone,pPF in the deep zone,and pTr in the medial superficial zone(P<0.05).The T2 value of the articular cartilage on the affected side of aTr at the superficial part of the lateral side and AT in medial deep zone was higher than that on the contralateral side(P<0.05).Compared to the superficial and deep articular cartilage T2 values at the af-fected medial and lateral side,the superficial articular cartilage T2 values were significantly higher than the deep ones at all sites except for the lateral pPF and medial pTr(P<0.05).The thickness of pTr,PT articular cartilage on the affected side of the me-dial side was lower than that on the normal side.All these differences were statistically significant(P<0.05).Conclusion Pa-tients with ACL injury have increased T2 relaxation time and thinning of articular cartilage on several sites on the affected side compared to the contralateral side.ACL injury is an important cause of the development of PTOA.
