摘要
目的 探讨三维能量超声联合彩色多普勒超声检查对胎儿宫内窘迫的评估意义.方法 选取妊娠大于 28周孕妇宫内缺氧的 30 例胎儿作为观察组,另选 50 例正常胎儿作为正常对照组,分别测量两组胎儿大脑中动脉(MCA)、脐动脉(UA)、孕妇左侧子宫动脉(LUA)的胎儿脐动脉收缩压与舒张压的比值(S/D)、血流灌注指数(PI)、血管阻力指数(RI)值以及胎盘三维能量多普勒血管化指数(VI)、血流指数(FI)、血管化血流指数(VFI),组间及组内进行对照分析.将产后结果作为对照,对比分析三维能量超声、彩色频谱多普勒超声诊断符合率.结果 观察组大脑中动脉S/D、PI、RI测值低于对照组,子宫动脉、脐动脉S/D、PI、RI测值高于对照组,差异有统计学意义(P<0.05);观察组胎盘VI、FI、VFI测值低于对照组,差异有统计学意义(P<0.05);彩色频谱多普勒超声确诊 24 例,三维能量超声确诊 29 例,对比差异有统计学意义(P<0.05),联合检查结果与产后结果完全一致.结论 三维能量超声测值评估胎儿宫内窘迫的灵敏度略高于彩色频谱多普勒超声测值,二者联合可提升评估准确性.
Abstract
Objective To explore the significance of three-dimensional energy ultrasound combined with spectral Doppler ul-trasound measurement in evaluating fetal intrauterine distress.Methods Pregnant women with gestational age greater than 28 weeks from December 2022 to January 2024 in our hospital were selected.Intrauterine hypoxia fetuses were classified as the ob-servation group(30 cases),while normal fetuses were selected as the normal control group(50 cases).The S/D,PI,RI values of the middle cerebral artery(MCA),umbilical artery(UA),left uterine artery(LUA)of the two groups of fetuses were measured,as well as the three-dimensional energy Doppler vascularization index(VI),blood flow index(FI),and vascularization blood flow index(VFI)of the placenta.Comparative analysis was conducted between groups and within groups.Using the output results as a comparison,we compared and analyzed the diagnostic accuracy of three-dimensional energy ultrasound and spectral Doppler ul-trasound.Results The MCAS/D,PI,and RI values in the observation group were lower than those in the control group,while the UA、LUAS/D,PI,and RI values were higher than those in the control group,with statistical significance(P<0.05);The VI,FI and VFI values in the observation group were lower than those in the control group,and the difference was statistically signifi-cant(P<0.05);24 cases were diagnosed by spectral Doppler ultrasound(diagnostic accuracy rate of 80.00%),and 29 cases were diagnosed by three-dimensional energy ultrasound(diagnostic accuracy rate of 96.67%),with statistically significant differences in comparison(χ2=4.043,P<0.05),the joint inspection results were completely consistent with the output results.Conclusion The sensitivity of three-dimensional energy ultrasound measurement in evaluating fetal intrauterine distress is slightly higher than that of spectral Doppler ultrasound measurement,and the combination of the two can improve the accuracy of the evaluation.
