超声引导下注射治疗脑卒中后肩痛综合征的临床应用

摘要
目的 评估超声引导下注射治疗脑卒中后肩痛综合征(SHS)的临床效果.方法 选取我院就诊的 100 例脑卒中后SHS患者,采用计算机随机分为观察组(超声引导下注射)与对照组(借助体表标志注射),每组患者 50 例.对比两组治疗前后的肩关节功能、生活质量及不良反应发生情况.结果 重复测量方差分析结果显示,mRs评分与肩关节功能评分的组间、时间及交互效应差异有统计学意义(P<0.01);组内比较,治疗前后不同时间的mRs评分与肩关节功能评分存在差异,随着治疗时间的延长mRs评分下降,肩关节功能评分升高,差异有统计学意义(P<0.01);组间比较,观察组治疗第 1 个星期、第 1 个月和第 3 个月的mRs评分低于对照组,肩关节功能评分均高于对照组,差异有统计学意义(P<0.01);交互效应:mRs评分与肩关节功能评分的组间效应与时间效应存在交互作用(P<0.01).两组不良反应发生率差异无统计学意义(P>0.05).结论 超声引导下注射治疗能够有效改善脑卒中后SHS患者的疼痛症状、上肢功能和生存质量,效果优于传统借助体表标志进行的注射治疗.
Abstract
Objective To evaluate the clinical effect of ultrasound-guided injection in the treatment of shoulder-hand syn-drome(SHS)after stroke.Methods 100 patients with shoulder pain after stroke aged 40-75 years in our hospital were retro-spectively selected,including 64 males and 36 females.They were randomly divided into observation group(injection guided by ultrasound)and control group(injection with anatomical markers),with 50 patients in each group.The shoulder joint function,quality of life and adverse reactions were compared between the two groups before and after treatment.Results The results of repeated measurement ANOVA showed that there were statistical differences among groups,time and interaction effects of mRs scores and shoulder joint function scores(P<0.01).Intra-group comparison showed differences between mRs Scores and shoul-der joint function scores at different times before and after treatment.With the extension of treatment time,mRs Scores decreased and shoulder joint function scores increased(P<0.01).The mRs Scores of the observation group were lower than those of the con-trol group at 1 week,1 month and 3 months after treatment,and the shoulder joint function scores were higher than those of the control group(P<0.01).As for the interaction effect,the intergroup effect of mRs Score and shoulder joint function score inter-acted with the time effect(P<0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Ultrasound-guided injection therapy can effectively improve the pain symptoms,upper limb function and quality of life of patients with SHY after stroke,and the effect is better than that of traditional injection therapy with body surface markers.
