多层螺旋CT与X线摄影检查在人工髋关节置换术中的应用价值

摘要
目的 探讨多层螺旋CT与X线摄影检查在人工髋关节置换术中应用价值.方法 选取我院疑似髋关节病变患者 221 例,均行多层螺旋CT及X线摄影检查,分析两种检查结果.结果 221 例疑似髋关节病变患者中,多层螺旋CT检查确诊需手术的患者 193 例,X线摄影检查确诊的为 172 例,多层螺旋CT检查特异度、敏感度、准确率分别为92.59%、99.48%、98.64%,高于X线摄影检查的 91.01%、43.75%、84.16%,差异有统计学意义(P<0.05);以手术结果为金标准,多层螺旋CT在髋部骨折、关节囊积液、关节周围软组织肿胀检出率为 100%、98.08%、88.24%,高于X线摄影检查的 45.16%、90.53%、47.06%,差异有统计学意义(P<0.05).结论 人工髋关节置换术前多层螺旋CT比X线摄影检查具有更高的准确率.
Abstract
Objective To explore the application value of multi-slice spiral CT and X-ray preoperative examination in artifi-cial hip replacement surgery.Methods 221 suspected patients with hip joint disease in our hospital were selected,all of whom underwent multi-slice spiral CT and X-ray instrument examinations,and the results of the two examinations were com-pared.Result Among the 221 suspected patients with hip joint lesions,193 cases were diagnosed by multi-slice spiral CT and 172 cases were diagnosed by X-ray imaging as requiring surgery.The specificity,sensitivity,and accuracy of spiral CT ex-amination were 92.59%,99.48%and 98.64%,respectively,higher than 45.16%,90.53%and 84.16%by X-ray examina-tion,correspondingly,with P<0.05;using surgical results as the gold standard,the detection rates of hip fractures,joint cap-sule effusion,and soft tissue swelling around the joints were 100%,98.08%and 88.24%,respectively,higher than 94.74%,65.38%and 47.06%by X-ray examination,with P<0.05.Conclusion Multi slice multi-slice spiral CT has higher accuracy than X-ray imaging before artificial hip replacement surgery.
