摘要

目的 探讨最佳单能量CTA重建技术在皮瓣移植术前评估受区血管的价值.方法 选取本院烧伤科收治的游离皮瓣移植患者 15 例,手术前均行创面部位能谱CTA检查,选取最佳单能量图像进行去骨伪影重建、容积再现(VR)、最大密度投影(MIP)、曲面重建(CPR)、多平面重建(MPR)全方位观察创面目标血管情况,寻找目标血管显示最佳平面,测量目标血管长度(mm)及分、最小管径(mm)、与毗邻骨骼关系,对应体表标志距离(mm).结果 15 例中有 12 例患者最佳单能量CTA图像能清楚显示目前血管情况,3 例患者因运动伪影及金属伪影影响,图像质量不佳,无法明确显示目标血管情况.结论 高质量的最佳单能量CTA图像能清楚显示创面目标血管情况,为临床游离皮瓣移植手术的血管吻合提供明确的指导意义,减少受区软组织创伤,缩短手术时间,提高皮瓣存活率,减少患者精神及经济压力.

Abstract

Objective To explore the he value of optimal single energy CTA in evaluating the vascular condition of the recipi-ent area before skin flap transplantation.Methods A retrospective analysis was conducted on 15 patients with high-voltage electric burn skin flap transplantation admitted to the burn department.The energy spectrum CTA examination on the wound site was performed,and the optimal single energy image was selected for bone removal artifact reconstruction,volume rendering(VR),maximum density projection(MIP),curved surface reconstruction(CPR),and multiplanar reconstruction(MPR)to ob-serve the target blood vessels in all directions.The optimal plane for displaying the target blood vessels was found,and the length and branches of the target blood vessels were measured,as well as the minimum diameter(mm),relationship with adjacent bones,and corresponding body surface marker distance(mm).Results Among them,12 patients had the best single energy image that could clearly display the current vascular condition,while 3 patients had poor image quality due to motion artifacts and metal artifacts,which prevented them from displaying the target vascular condition.Conclusion The optimal single energy CTA image can clearly display the target blood vessels on the wound surface,thereby providing clear guidance for vascular anas-tomosis in clinical free flap transplantation surgery,reducing trauma to the affected area,and shortening surgical time.
