摘要
目的 探讨经颅黑质超声检查及MRI在帕金森病诊断评估中的价值.方法 选取我院帕金森病患者 121 例设为观察组、同期健康体检者 121 例设为对照组,均接受经颅黑质超声检查及MRI检查,统计观察组与对照组、观察组不同Hoehn-Yahr分级患者经颅黑质超声参数[黑质高回声总面积和中脑总面积比值(S/M)、黑质高回声面积]及MRI参数[中脑直径、黑质致密带宽度(WPCSN)、WPCSN/中脑直径)],分析经颅黑质超声检查及MRI参数与Hoehn-Yahr分级的相关性.结果 观察组S/M、黑质高回声面积高于对照组,WPCSN、WPCSN/中脑直径低于对照组(P<0.05).观察组不同Hoehn-Yahr分级患者S/M、黑质高回声面积、WPCSN、WPCSN/中脑直径间差异有统计学意义(P<0.05).Spearman检验证实S/M、黑质高回声面积与Hoehn-Yahr分级呈正相关性,WPCSN、WPCSN/中脑直径与Hoehn-Yahr分级呈负相关性(P<0.05).结论 通过经颅黑质超声检查及MRI检查可明确帕金森病患者颅脑特征,且不同Hoehn-Yahr分级帕金森病类型患者经颅黑质超声检查及MRI参数存在差异.
Abstract
Objective This study aimed to investigate the value of transcranial sonography(TCS)of the substantia nigra(SN)and magnetic resonance imaging(MRI)in the diagnosis of Parkinson's disease(PD).Methods Overall,121 patients with PD from our hospital and 121 healthy individuals who underwent medical checkups in the same period were selected as the observation and control groups,respectively,all of whom received TCS of the SN and MRI.The TCS(ratio of total hyperechoic substantia nigra area to total midbrain area,S/M),hyperechoic substantia nigra area and MRI parameters(midbrain diameter,width of pars compacta of substantia nigra,WPCSN),WPCSN/midbrain diameter were collected for the analysis of the relation-ship between the Hoehn-Yahr scale and TCS parameters of the SN and MRI.Results The observation group had higher S/M and total hyperechoic substantia nigra area and lower WPCSN and WPCSN/midbrain diameter values than the control group(P<0.05).The S/M,total hyperechoic substantia nigra area,WPCSN,and WPCSN/midbrain diameter values had significant differ-ences between patients of different Hoehn-Yahr scale in the observation group(P<0.05).Spearman's test confirmed a positive correlation between S/M,total hyperechoic substantia nigra area,and Hoehn-Yahr scale and a negative correlation between WPCSN,WPCSN/midbrain diameter,and Hoehn-Yahr scale(P<0.05).Conclusion The cranial characteristics of patients with PD can be distinguished by TCS of the SN and MRI,and its parameters show significant differences between patients with PD of different Hoehn-Yahr scale.
