输尿管结石嵌顿的CT预测价值初步探讨

摘要
目的 探讨术前输尿管结石嵌顿的CT预测因素,进一步提高术前预测的准确性.方法 选取 81 例输尿管结石患者的影像学资料.根据结石嵌顿定义将患者分为嵌顿组和非嵌顿组,分别记录性别、年龄、结石部位;在轴位CT图像评价结石的最大径、CT值,结石部位输尿管壁厚度,结石上下层面输尿管最大径及CT值,冠状位图像测量结石最大径.两组间数据比较,差异有统计学意义(P<0.05).结果 结石嵌顿组 38 例,结石非嵌顿组 43 例.结石嵌顿组与结石非嵌顿组比较:轴位结石的最大径、结石CT值,结石部位输尿管壁厚度,结石上方层面输尿管最大径、结石下方层面CT值差异有统计学意义(P<0.05),患者年龄、结石位置、结石下方层面输尿管最大径、结石上方层面输尿管CT值两组间差异无统计学意义(P>0.05).结论 术前CT平扫预测输尿管结石嵌顿是可行的,尤其是结石大小及密度、结石处输尿管壁厚度、结石上方层面输尿管最大径、结石下方层面输尿管CT值可作为重要术前预测因素.
Abstract
Objective To explore the predictive factors of impacted ureteral stone with CT before operation,and to further improve the accuracy of preoperative prediction.Methods CT imaging materials of 81 patients with ureteral stone were ana-lyzed retrospectively.All patients were divided into impacted ureteral stone group and non-impacted ureteral stone group based on the definition of impcted ureteral stone.Gender,age,stone location were recorded.Axial CT images were used to evaluate the maximum diameter and CT value of stone,thickness of ureteral wall at the site of stone,maximum diameter and CT value of ure-ter at upper and lower layers of stone,and coronal images were used to measure the maximum diameter of the stone.Comparing the data of the two groups,P<0.05 was considered statistically significant.Results All 81 patients were included in the study,including 38 cases in the impacted ureteral stone group and 43 cases in the non-impacted ureteral stone group.Comparing the two groups,it was found there were statistically significant differences in the maximum diameter and CT value of stone in axial posi-tion,thickness of ureteral wall at the site of stone,the maximum diameter of ureteral at the upper layer of stone,and the CT value of ureter at the lower layer of stone.There were no statistically significant differences in age,location of stone,the maxi-mum diameter of ureter at lower layer of stone,and the CT value of ureter at upper layer of stone.Conclusion It is feasible to predict impacted ureteral stone by preoperative CT,especially the size and density of stone,the thickness of ureteral wall at the stone site,the maximum diameter of ureter at upper layer of stone,and the CT value of ureter at lower layer of stone,which can be used as important preoperative predictors.
