摘要
目的 探讨定量磁化率图(QSM)对偏头痛患者大脑深部核团中的铁代谢改变及其与认知和情绪改变的关系.方法 选取 38 例偏头痛患者(MIG组)和 41 例健康查体者(HCs组),使用磁共振序列QSM观察NAC、DN、EXA、SNc、RN、SNr、PBP、HTH及STH铁沉积变化,并观察其与认知、情绪及偏头痛特征的相关性.结果 MIG组表现出多种认知功能的损害,MIG组的NAC(P=0.032)、EXA(P=0.035)、SNc(P=0.019)、RN(P=0.014)和SNr(P=0.010)的QSM值均高于HCs组.此外,NAC(r=0.439,P=0.008)、EXA(r=0.512,P=0.002)的QSM值与TMT-A评分呈正相关;RN(r=0.448,P<0.007)的QSM值与MSQ评分呈正相关;SNr(r=0.448,P<0.007)的QSM值与PHQ9 评分呈正相关.结论 定量磁化率图(QSM)为偏头痛患者认知功能改变的分析提供了参考.
Abstract
Objective To investigate the changes in iron metabolism in deep brain nuclei of migraine patients using quantita-tive magnetic susceptibility mapping(QSM)and their relationship with cognitive and emotional changes.Methods This study included 38 migraine patients(MIG)and 41 healthy controls(HCs).Using magnetic resonance sequences QSM,we investi-gated the changes in iron deposition in the nucleus accumbens(NAC),cerebellar dentate nucleus(DN),extended amygdala(EXA),substantia nigra pars compacta(SNc),red nucleus(RN),substantia nigra reticular region(SNr),peribrachial pig-ment nuclei(PBP),hypothalamus(HTH),and subthalamic nucleus(STH),as well as observed their correlation with cogni-tive,emotional,and migraine characteristics.Results The MIG group exhibited various cognitive impairments.The QSM val-ues of NAC(P=0.032),EXA(P=0.035),SNc(P=0.019),RN(P=0.014),and SNr(P=0.010)in the MIG group were higher than those in the HC group.In addition,the QSM values of NAC(r=0.439,P=0.008)and EXA(r=0.512,P=0.002)were posi-tively correlated with the TMT-A score.The QSM value of RN(r=0.448,P<0.007)was positively correlated with the MSQ score.The QSM value of SNr(r=0.448,P<0.007)was positively correlated with PHQ9 score.Conclusion This study provides a new perspective for analyzing cognitive and emotional changes in migraine patients.
