MRI DTI参数与缺血性脑梗死患者神经功能的关系及对预后的评估价值分析

摘要
目的 分析磁共振成像(MRI)弥散张量成像(DTI)参数与缺血性脑梗死(ICI)患者神经功能的关系及对预后的评估价值.方法 选取本院 125 例 ICI患者资料,均接受 MRI DTI检查,依据美国国立卫生研究院卒中量表(NIHSS)评分评估神经功能损伤,分为轻型组、中型组与重型组,分析MRI DTI参数[衰减指数(Exat)、容积比各向异性(VRA)、平均弥散系数(DCavg)及各向异性指数(FA)]与神经功能缺损相关性;根据 3 个月后预后情况,分为预后不良组和预后良好组,比较两组MRI DTI参数,绘制受试者工作特征(ROC)曲线分析MRI DTI参数对预后评估价值.结果 轻型组、中型组与重型组Exat、DCavg比较差异无统计学意义(P>0.05);轻型组与中型组VRA、FA显著高于重型组,且轻型组显著高于中型组,差异有统计学意义(P<0.05);Exat、DCavg与神经功能缺损差异无统计学意义(P>0.05),VRA、FA与神经功能缺损程度差异有统计学意义(P<0.05);预后不良组VRA、FA显著低于预后良好组,差异有统计学意义(P<0.05);VRA、FA及二者联合评估患者预后曲线下面积(AUC)分别为 0.720、0.877、0.903,联合评估敏感度为 92.14%,特异度为 72.40%.结论 VRA、FA与ICI患者神经功能密切相关,二者联合对患者预后具有较高评估效能.
Abstract
Objective To analyze the relationship of magnetic resonance imaging(MRI)diffusion tensor imaging(DTI)pa-rameters with neurological function in patients with ischemic cerebral infarction(ICI),and to evaluate their prognostic value.Methods The data of 125 patients with ICI who received MRI DTI were analyzed.According to the National Institutes of Health Stroke Scale(NIHSS)score,the patients were divided into mild group,moderate group and severe group.The correlation be-tween MRI DTI parameters,in terms of exponential attenuation(Exat),volume ratio anisotropy(VRA),average diffusion coef-ficient(DCavg),as well as fractional anisotropy(FA),and neurological impairment,was analyzed.According to the 3-month prognosis,the patients were divided into poor prognosis group and good prognosis group.The MRI DTI parameters of the two groups were comparatively analyzed,and ROC curve was used to analyze the prognostic value of these MRI DTI parameters.Results There was no significant difference in Exat or DCavg among the mild group,the moderate group and the severe group(P>0.05).VRA and FA decreased in order from the mild group,the moderate group to the severe group(P<0.05).There was no significant correlation between Exat,DCavg and neurological impairment(P>0.05),while,VRA and FA were negatively corre-lated with the degree of neurological impairment(P<0.05).VRA and FA in the poor prognosis group were significantly lower than those in the good prognosis group(P<0.05).The area under the ROC curve(AUC)values of VRA,FA and their combina-tion for prognosis was 0.720,0.877 and 0.903,respectively.The sensitivity and specificity of combined evaluation were 92.14%and 72.40%.Conclusion VRA and FA are closely related to the neurological function in patients with ICI.The combination of the two can achieve better performance in the prognosis of patients with ICI.
