摘要
目的 探讨应用放射性核素平衡法心血池显像评价冠心病心力衰竭患者左心功能的临床应用价值.方法 选取本院就诊的冠心病患者 89 例,其中男性 48 例,女性 41 例,平均年龄(70.0±11.4)岁.分为冠心病心衰组 36 例,其余 53 例为冠心病组,均进行核素平衡法心血池显像.比较两组患者左心功能参数LVEF、PER、PFR、MFR的水平变化,并分析冠心病心衰患者左心功能各参数水平两两之间的相关性.结果 与冠心病组相比,冠心病心衰组患者年龄偏大,并在糖尿病、慢性肾功能不全及BNP水平上显著增高,差异具有统计学意义(P<0.05,P<0.01).两组患者在性别、吸烟、高血压、高血脂、脑卒中、慢性阻塞性肺病水平比较差异均无统计学意义(P>0.05).冠心病心衰组较冠心病组的左心功能参数LVEF、PER、PFR、MFR水平均显著减低,差异有统计学意义(P<0.01,P<0.001),并且冠心病心衰组中LVEF、PER、PFR、MFR水平两两之间均具有显著相关性(P<0.01,P<0.001).结论 应用放射性核素平衡法心血池显像可有效评估冠心病心力衰竭患者的左心功能参数水平变化,为临床诊疗提供早期的指导价值.
Abstract
Objective To explore the clinical value of radionuclide cardiac blood pool imaging in evaluating left ventricular function in patients with coronary heart disease and heart failure.Methods A total of 89 patients with coronary heart disease were selected,including 48 males and 41 females,with an average age of(70.0±11.4)years.36 patients were divided into coronary heart disease and heart failure group,and the remaining 53 patients were divided into coronary heart disease group.All patients underwent nuclide cardiac blood pool imaging.We compared the levels of left ventricular function parameters LVEF,PER,PFR,MFR between the two groups of patients,and analyzed the correlation between the levels of left ventricular function parameters in patients with coronary heart disease and heart failure.Results Compared with the coronary heart disease group,the patients in the coronary heart disease and heart failure group were older and significantly increased in diabetes,chronic renal insufficiency and BNP levels(P<0.05,P<0.01).There was no significant difference in gender,smoking,hypertension,hyper-lipidemia,stroke,and chronic obstructive pulmonary disease between the two groups(P>0.05).The levels of left ventricular function parameters,i.e.,LVEF,PER,PFR,and MFR,in the coronary heart failure group were significantly lower than those in the coronary heart disease group(P<0.01,P<0.001),and there was a significant correlation between the levels of LVEF,PER,PFR,and MFR in the heart failure group(P<0.01,P<0.001).Conclusion Radionuclide cardiac blood pool imaging can effectively assess the level of left ventricular function parameters in patients with coronary heart disease and heart failure,providing early guiding value for clinical diagnosis and treatment.
