MSCTPA、D-D与修正Geneva量表对肺栓塞的预测价值探讨

摘要
目的 探讨多排螺旋CT肺动脉造影(MSCTPA)、D-二聚体(D-D)与修正Geneva量表对肺栓塞(PE)的预测价值.方法 选取本院就诊的 85 例疑似PE患者,均在本院接受MSCTPA检查,并检测患者D-D水平,评估其修正Geneva量表评分.绘制受试者工作特征(ROC)曲线比较三者对于PE的预测价值,分析MSCTPA参数、D-D以及修正Geneva评分的相关性.结果 MSCTPA对于PE的诊断准确率及Kappa一致性均高于D-D和修正Geneva评分,差异有统计学意义(P<0.05);PE组患者CTPA的PE指数(CTI)、D-D水平以及修正Geneva评分均显著高于非PE组的CTI值、D-D水平和修正Geneva评分,差异有统计学意义(P<0.05);CTI、D-D水平以及修正Geneva评分三者联合诊断PE的AUC值、特异度均显著高于单一诊断;CTI、D-D以及修正Geneva评分三者之间互为正相关,差异有统计学意义(P<0.05).结论 MSCTPA、D-D与修正Geneva量表评分均可应用于疑似PE的诊断中,具有一定的诊断价值,但单一诊断并不能完全排除PE诊断,三者联合诊断PE的特异度更高.
Abstract
Objective To study the predictive value of multi-slice CT pulmonary angiography(MSCTPA),D-dimer(D-D)and modified Geneva scale for pulmonary embolism(PE).Methods A total of 85 suspected PE patients admitted to our hospi-tal were selected as the study subjects,and their clinical data were retrospectively analyzed.All patients were examined by MSCTPA in our hospital,their D-D level was tested,and their modified Geneva scale score was assessed.Receiver operating characteristic(ROC)curve was used to compare the predictive value for PE,and the correlation of MSCTPA parameter,D-D and modified Geneva score was analyzed.Results diagnostic accuracy and Kappa consistency of MSCTPA for PE were higher than D-D and modified Geneva scores(P<0.05);PE index(CTI),D-D level and modified Geneva score of CTPA,in PE group were significantly higher than those in non-PE group(P<0.05);The AUC value and sensitivity of CTI,D-D level and modified Geneva score in combined diagnosis of PE were significantly higher than those in single diagnosis(P<0.05);CTI,D-D and modified Geneva scores were positively correlated(P<0.05).Conclusion MSCTPA,D-D and modified Geneva scale scores can all be applied to the diagnosis of suspected PE and have certain diagnostic value.However,a single diagnosis cannot com-pletely exclude the diagnosis of PE,and the combined diagnosis of PE has higher specificity.
