猫抓病性淋巴结炎的影像学表现

摘要
目的 探讨猫抓病性淋巴结炎的影像学诊断价值.方法 回顾性分析 4 例经临床病理证实的猫抓病性淋巴结炎患者的CT、MRI表现并进行文献复习.结果 4 例均为多发肿大淋巴结,边界清楚,短径约 1.4～2.4 cm,CT特征呈均匀或不均匀软组织密度结节,MRI呈均匀或不均匀T1WI等信号、T2WI高信号结节,1 例CT及MRI增强表现为不均匀明显强化,1 例MRI增强表现为"玫瑰花"征;3 例广泛皮下脂肪水肿.结论 猫抓病性淋巴结炎影像学表现有一定特征性,结合临床有猫等动物密切接触史或抓、咬伤史,可提示猫抓病性淋巴结炎诊断.
Abstract
Objective To explore the value of imaging findings of lymphadenitis associated with cat-scratch disease.Meth-ods With the literature reviewed,CT and MRI images of 4 patients confirmed by pathology with lymphadenitis associated with cat-scratch disease were retrospectively analyzed.Results All four patients presented as multiple well-circumscribed enlarged lymph nodes.The short diameter of enlarged lymph nodes ranged from 1.4 to 2.4 cm.CT exhibited homogeneous or inhomoge-neous isodensity to muscle.MRI showed homogeneous or inhomogeneous isointensity on T1WI and hyperintensity on T2WI com-pared with muscle.After the administration of contrast agent,CT and MRI showed heterogeneously marked enhancement in one patient,MRI showed"rose-flower"sign in another patient.Extensive subcutaneous edema was observed in three patients.Con-clusion Features of imaging findings can be assessed in the lymphadenopathy caused by cat-scratch disease.With the combina-tion of imaging findings and the history of cat scratching,accurate diagnosis could be obtained.
