血友病性假肿瘤的临床与影像学分析

摘要
目的 探讨血友病性假肿瘤(HP)患者的临床和影像学特征.方法 选取 24 例经临床、影像学及实验室检查确诊的血友病性假肿瘤患者的临床及影像学资料.结果 24 例患者中甲型 22 例,乙型 2 例;轻型 4 例,中型 2 例,重型18 例.重型、轻型及中型患者之间外伤发生率差异无统计学意义(P>0.05).血友病性假肿瘤高发年龄为 20～39 岁,占总血友病性假肿瘤人数的 45.8%.24 例患者共发现 38 个病灶,其中软组织型 23 个病灶,骨骼内型 12 个病灶,骨膜下型3 个病灶;主要影像学表现为软组织肿块,单发或多发囊样或膨胀性骨质破坏、破坏边缘骨质硬化、骨膜反应及骨皮质受压吸收.部分病变存在点状及不规则形钙化.结论 血友病性假肿瘤好发于青壮年血友病患者,其具有特定的影像学特征,结合病史可以明确诊断.
Abstract
Objective To illustrate the variable radiologic appearance of hemophilic pseudotumors in musculoskeletal sys-tems as shown by radiography,CT and MRI.Methods The clinical feature and the imaging findings of 24 patients with hemo-philia were analyzed in this retrospective study.Results In 24 cases of hemophilic pseudotumors,there were 22 cases of hemo-philia A and 2 cases of hemophilia B.There was no statistical significance in the incidence of trauma among severe patients,mild patients and moderate patients(P>0.05).There were 23 soft-Tissue lesions,3 subperiosteal lesions,and 12 intraosseous lesions of all the 24 patients.Main image findings included soft tissue swelling or lump,bone destruction with edge sclerosis,osteolysis lesion.Conclusions Hemophilic pseudotumor has radiographically variable appearances.Hemophilic history of patients can contribute to the right diagnosis of pseudotumor.
