多静息态fMRI指标在MRI阴性颞叶癫痫患者大脑功能网络中的研究

摘要
目的 探讨MRI阴性颞叶癫痫(MRIn-TLE)患者大脑功能网络的异常变化.方法 选取符合纳入标准的患者MRIn-TLE组、海马硬化型内侧颞叶癫痫(MTLE-HS)组及健康志愿者(HC组)的静息态fMRI数据,预处理后计算各组的低频振幅(ALFF)、低频振幅分数(fALFF)及局部一致性(ReHo),并对各组静息态指标进行统计学分析.结果 相比HC组,MRIn-TLE组的异常功能脑区呈弥漫性分布,主要表现为豆状壳核、三角部额下回及中央旁小叶的ALFF值,扣带回、补充运动区、中央后回及角回的fALFF值,距状裂周围皮层、颞下回及楔叶的ReHo值均升高;背外侧额上回、额中回、楔前叶及小脑的ALFF值,小脑、颞下回及颞中回的fALFF值,背外侧额上回及楔前叶的ReHo值均降低.MTLE-HS组相较于HC组,异常脑区的分布以顶叶激活为主,表现为楔前叶的ALFF、fALFF和ReHo值升高.结论 MRIn-TLE组患者多个脑区的静息态fMRI指标与对照组相比存在差异,且检出存在差异的脑区较MTLE-HS组患者分布更为广泛,主要包括颞叶、额叶、枕叶及顶叶.
Abstract
Objective To investigate the abnormal changes of brain functional network in patients with MRI-negative tempo-ral lobe epilepsy.Methods The resting-state fMRI data of MRIn-TLE patients,MTLE-HS patients and healthy volunteers(HC group)were collected.After preprocessing,the amplitude of low-frequency fluctuation(ALFF),fractional amplitude of low-frequency fluctuation(fALFF)and regional homogeneity(ReHo)of each group were calculated,and the rs-fMRI parameters of each group were statistically analyzed.Results Compared with the HC group,the MRIn-TLE group showed a diffuse distribu-tion of abnormal brain regions,mainly showing the increased ALFF values in the putamen,inferior frontal gyrus and paracentral lobules in the triangle,the increased fALFF values in the cingulate gyrus,supplementary motor area,postcentral gyrus and an-gular gyrus,and the increased ReHo values in the cortex around the calcarine cleft,inferior temporal gyrus and cuneus.The ALFF values of the dorsal-lateral superior frontal gyrus,middle frontal gyrus,precuneus and cerebellum,the fALFF values of the cerebellum,inferior temporal gyrus and middle temporal gyrus,and the ReHo values of the dorsal-lateral superior frontal gyrus and precuneus all decreased.Compared with the HC group,the MTLE-HS group showed the increased ALFF,fALFF and ReHo values in the precuneus,mainly in the parietal lobe.Conclusion The resting-state fMRI parameters of multiple brain regions in MRIn-TLE patients were different from those in the control group,and the brain regions with differences were more widely distrib-uted than those in MTLE-HS patients,mainly including temporal lobe,frontal lobe,occipital lobe and parietal lobe.
