MRI在松果体区乳头状瘤与生殖细胞瘤术前鉴别诊断中的应用

摘要
目的 探讨影像学在术前鉴别诊断松果体区乳头状瘤(PTPR)与生殖细胞瘤的临床价值.方法 选取北京天坛医院经手术病理确诊的 17 例PTPR及 32 例单发松果体区生殖细胞瘤的影像学资料,总结其MRI特征.结果 PTPR发生年龄较生殖细胞瘤年龄稍大,男女无明显差异.影像学特点有显著差异:与生殖细胞瘤比较,PTPR在CT表现钙化少见,MRI在T1WI常呈高信号,在T2WI多呈混杂信号,增强后呈不均匀强化.结论 松果体区生殖细胞瘤及PTPR有其影像学特点,术前若明确诊断,对临床治疗有指导意义.
Abstract
Objective To investigate the clinical value of imaging in the differential diagnosis of papillary tumor of the pineal region(PTPR)and germinoma before operation.Methods The imaging data of 17 cases of PTPR and 32 cases of single pineal region germinoma confirmed by surgery and pathology in Beijing Tiantan Hospital were selected and their characteristics were summarized.Results The age of PTPR was slightly older than that of germinoma,and there was no significant difference be-tween male and female.There were significant differences in imaging features.Compared with germinoma,PTPR calcification was rare on CT,PTPR showed high signal intensity more commonly on T1WI;most of them showed mixed signal intensity on T2WI;and PTPR showed heterogeneous enhancement after enhancement.Conclusion Pineal region space occupying germinoma and PTPR have their imaging features.If the diagnosis is confirmed before operation,it has guiding significance for clinical treatment.
