摘要
目的 探讨MRI评分法在腮腺良性肿瘤鉴别诊断中的价值.方法 选取 137 例 145 枚腮腺良性肿瘤的MRI影像资料,包括 58 例 58 枚多形性腺瘤(pleomorphic adenoma,PA)、54 例 62 枚腺淋巴瘤(warthin's tumor,WT)和 25 例25 枚基底细胞腺瘤(basal cell adenomas,BCA),将瘤体位于尾叶、形态规则、T2WI等低信号、T1WI高信号和快进快出强化模式赋予 0 分,瘤体位于非尾叶、形态不规则、T2WI高信号、T1WI等低信号和渐进性强化模式赋予 2 分,采用受试者操作特性曲线评价MRI评分鉴别PA、WT和BCA的价值.结果 WT和PA的MRI评分分别为 2(0,2)分和 8(6,10)分(Z=10.0348,P<0.05),鉴别两者的曲线下面积(the area under curve,AUC)、最佳阈值、敏感度和特异度分别为 0.980、5 分、93.5%和 86.6%;WT和BCA的MRI评分分别为 2(0,2)分和 4(4,6)分(Z=-3.922,P<0.05),鉴别两者的AUC、最佳阈值、敏感度和特异度分别为 0.866、3 分、79.0%和 84.0%;PA和BCA的MRI评分分别为 8(6,10)分和 4(4,6)分(Z=3.778,P<0.05),鉴别两者的AUC、最佳阈值、敏感度和特异度分别为 0.866、7 分、80.0%和 74.1%.结论 WT评分最低,PA评分最高,BCA介于两者间.MRI评分法对腮腺良性肿瘤的鉴别诊断有重要价值.
Abstract
Objective To explore the value of MRI scoring in the differential diagnosis of benign parotid tumors.Methods The MRI images of 145 benign parotid tumors in 137 cases were retrospectively analyzed.There were 58 pleomorphic adenoma(PA)in 58 cases,62 Warthin's tumor(WT)in 54 cases and 25 basal cell adenomas(BCA)in 25 cases.The tumor was located in the caudal lobe,with regular morphology,equal low signal on T2WI,high signal on T1WI,and fast in/out enhancement mode assigned a score of zero.The tumor was located in the no ncaudal lobe,irregular morphology,high signal on T2WI,equal low signal on T1WI,and progressive enhancement modes were assigned 2 points.The value of MRI scores in distinguishing PA,WT,and BCA was evaluated using the receiver operating characteristic curve.Results The MRI scores of WT and PA were 2(0,2)points and 8(6,10)points(Z=10.0348,P＜0.05),respectively.The area under curve(AUC),optimal threshold,sensi-tivity,and specificity for distinguishing the two were 0.980,5 points,93.5%,and 86.6%,respectively;The MRI scores of WT and BCA were 2(0,2)and 4(4,6)points(Z=-3.922,P<0.05),respectively.The AUC,optimal threshold,sensitivity,and specificity for distinguishing the two were 0.866,3 points,79.0%,and 84.0%,respectively;The MRI scores of PA and BCA were 8(6,10)and 4(4,6)points(Z=3.778,P<0.05),respectively.The AUC,optimal threshold,sensitivity,and specificity for distinguishing the two were 0.866,7 points,80.0%,and 74.1%,respectively.Conclusion The MRI score is of great value in the differential diagnosis of benign parotid tumors.WT score is the lowest,PA score is the highest,and BCA is in between.
