不同浓度对比剂和注射速率在锥光束乳腺CT中的应用比较

摘要
目的 分析碘对比剂浓度和注射速率对锥光束乳腺CT增强扫描(CE-CBBCT)病灶强化的影响,以期探讨较为理想的对比剂注射方案.方法 选取中山大学肿瘤防治中心 358 例临床诊断为乳腺癌的患者,均行CE-CBBCT.根据增强扫描所用碘对比剂浓度(370 mgI/ml,350 mgI/ml)不同分为 370 组 186 例和 350 组 172 例,观察病灶的强化效果.结果 静脉注射对比剂 2 min病灶的强化CT值>1 min病灶的强化CT值,差异有统计学意义(P<0.05).相同的注射速率,370 组和 350 组病灶的强化比较,差异无统计学意义(P>0.05).350 组和 370 组随着注射速率的增加,病灶强化逐渐增加,370 组病灶强化 CT 值差异无统计学意义(P>0.05),350 组病灶强化 CT 值差异有统计学意义(P<0.05).结论 2 min病灶的强化CT值优于 1 min.相同注射速率,350 组对比剂病灶强化 CT 值和 370 组比较差异无统计学意义(P>0.05).使用 370 mgI/ml对比剂可适当降低注射速率,使用 350 mgI/ml对比剂可适当加快注射速率.
Abstract
Objective To evaluate the effect of different concentration and injection flow rate of iodine contrast agent on the enhancement of CE-CBBCT lesions,in order to explore an ideal contrast agent injection scheme.Methods A total of 358 pa-tients with clinical diagnosis of breast cancer in Sun Yat-Sen University Cancer Center were collected,and all patients underwent enhanced breast CT scans.The patients were divided into 370 group and 350 group according to the concentration of contrast agent(370 mgI/ml,350 mgI/ml)used in enhanced scanning,and the enhancement effect of lesions was observed.Results The enhanced CT value of the lesion at 2 min after intravenous injection of contrast agent was greater than the enhanced CT value of the lesion at 1 min,and the difference was statistically significant(P<0.05).At the same injection speed,there was no signifi-cant difference in the enhancement of lesions between the 370 and 350 group(P>0.05).With the increase of injection speed,the enhancement of lesions in the 350 group and the 370 group gradually increased.There was no significant difference in the CT value of the lesion in the 370 group(P>0.05),and the difference in the enhanced CT value of the lesion in the 350 group was sta-tistically significant(P<0.05).Conclusion The enhanced CT value of 2 min lesions is better than 1 min.At the same injection speed,the contrast-enhanced CT value of the 350 group is the same as that of the 370 group.The use of 370 contrast agent can appropriately reduce the injection speed,and the use of 350 contrast agent can appropriately speed up the injection speed.
