摘要

目的 探讨磁共振成像(MRI)联合乳腺X线摄影(mammography,MG)对不伴肿块型乳腺钙化病变的诊断价值.方法 选取MG表现为不伴肿块型钙化且MRI呈肿块样强化的 139 例患者(142 个病灶)的影像学特征,将单因素分析有统计学意义的征象纳入logistic多因素分析,构建MG、MRI及联合检查的回归模型.结果 联合检查的模型中钙化形态、伴随征象,肿块的边缘、时间-信号强度曲线(time-signal intensity curve,TIC)及表观扩散系数(apparent diffusion coefficient,ADC)值是良恶性钙化的独立预测因素.MG、MRI及联合检查的回归模型曲线下面积(area under the curve,AUC)分别为 0.793、0.894、0.928.联合检查显著优于MG单独检查(P<0.05).结论 对于MG检出的不伴肿块型乳腺钙化病变,MG联合MRI能显著提高诊断效能.

Abstract

Objective To investigate the diagnostic value of MRI combined with mammography in breast lesions showing cal-cification without mass on mammography.Methods The imaging features of 139 patients(142 lesions)who were diagnosed with breast calcification without mass by MG,and mass-like enhancement on the MRI was retrospectively analyzed based on the pathological results,the statistically significant signs of univariate analysis were included in the multivariate logistic regression analysis.The regression model of MG,MRI and combined examination was established.Results The morphology of calcifica-tion and accompanying signs,margin,time-signal intensity curve(TIC)and apparent diffusion coefficient(ADC)were indepen-dent predictors of benign and malignant calcification in the model of combined examination.The area under the curve(AUC)of MG,MRI and combined examination was 0.793,0.894 and 0.928,respectively.Combined examination was significantly supe-rior to MG alone(P<0.05).Conclusion For breast lesions showing calcification without mass on mammography,MG com-bined with MRI can significantly improve the diagnostic accuracy.
