摘要
目的 探讨腺泡状软组织肉瘤(ASPS)的MRI表现.方法 选取经手术或穿刺病理证实的ASPS患者 13 例的临床资料,行MRI平扫及增强扫描,分析其MRI表现特征.结果 ASPS13 例患者中肿瘤位于下肢 12 例,上肢 1 例,最大径 11.5cm.5 例平扫T1WI呈稍低信号或低信号,2 例等信号,6 例稍高信号,13 例T2WI均呈高信号或稍高信号,DWI均显示高信号.增强扫描 9 例见不均匀明显强化,4 例见较均匀明显强化,13 例肿瘤的边缘和/或内部见流空血管影并见血管明显强化.7 例在初诊时有肺转移.结论 ASPS的临床和MRI表现有一定特异性,MRI信号特点、肿瘤边缘和/或内部的增粗迂曲血管影、增强明显强化是诊断ASPS的重要影像特征.
Abstract
Objective To investigate the MRI findings of alveolar soft tissue sarcoma.Methods The clinical data of 13 patients with alveolar soft tissue sarcoma confirmed by surgery or puncture pathology were selected.And then,MRI plain and enhanced scanning were performed and the MRI characteristics were analyzed.Results Among the 13 patients with alveolar soft tissue sarcoma,12 cases were found the tumors located in the lower limbs and the remaining one in the upper limbs,with a maximum diameter of 11.5 cm.Five cases showed slightly low or low signal,two cases showed equal signal,and six cases showed slightly high signal on T1WI.Thirteen cases showed hyperintensity or slight hyperintensity on T2WI.All 13 cases showed high signal on DWI.On contrast scanning,nine cases showed uneven and obvious enhancement,four cases showed more uni-form and obvious enhancement,and the edge and/or middle of the tumor presented an empty vascular shadow and obvious en-hancement of blood vessels in 13 cases.Seven patients had lung metastases at the time of initial diagnosis.Conclusion The clinical and MRI manifestations of ASPS have certain specificity,and the MRI signal characteristics,thickening and tortuous vascular shadows at the tumor margin and/or inside,and obvious enhancement are important imaging features for the diagnosis of ASPS.
