Based on literature analysis, this study systematically reviews the forms and basic content of metaphor research in contemporary clinical psychology. In the realm of measurement and diagnosis, the metaphorical abilities and characteristics of patients with specific disorders differ from those of healthy individuals, not due to metaphorical disorders. On one hand, their metaphorical understanding and creativity are influenced by factors such as familiarity with metaphors, ambiguity, and background information. On the other hand, they are constrained by their low non-verbal comprehension abilities, impaired executive functions, and compromised functions of relevant brain areas. In psychological counseling and therapy, metaphor serves as a linguistic phenomenon that helps elucidate emotions and constructs adaptive cognitive structures. Its integrative nature provides phenomenological evidence for counseling assessment, exhibiting distinct characteristics in different counseling stages.
