摘要
通过对维持治疗过渡到舒适照护的评估及决策、撤除维持生命治疗的流程以及可能影响撤除维持生命治疗的因素三个方面进行阐述.发现预估工具在一定程度上可以帮助医生判断是否需要进行关于生命终结话题的讨论,撤出维持生命治疗对家属及医务人员都是一个艰难的决定,可通过共享决策帮助家属、患者、医生、护士达成一致意见.同时,撤出维持生命治疗也面临着决策冲突、伦理困境等挑战,而且在具体临床实践中仍有很大差异,需要从医学、伦理、法律等多方面进行综合考量.
Abstract
This paper elucidates three aspects regarding the transition from life-sustaining treatment to comfort care,the process of withdrawing life-sustaining treatment,and factors that may influence the decision to withdraw life-sustaining treatment.It is found that the prognostic tools can help doctors to some extent determine whether discussions about end-of-life care are needed.Withdrawing life-sustaining treatment is a difficult decision for both family members and healthcare professionals,and shared decision-making can help achieve consensus among families,patients,doctors,and nurses.Additionally,withdrawing life-sustaining treatment faces challenges such as decision conflicts,ethical dilemmas,and significant variations in specific clinical practices,requiring comprehensive considerations from medical,ethical,legal,and other perspectives.
