摘要

目的 探讨应用扩张后颈横皮瓣修复儿童颈部病损的临床效果.方法 通过回顾空军军医大学西京医院整形外科2009年1月至2019年12月收治的颈部瘢痕挛缩的患儿,分析采用预扩张颈横皮瓣转移修复手术方法的可行性及优势.结果 13例患儿中,1例因扩张器置入术后伤口愈合不良导致扩张器外露而行清创、扩张器重置术;1例扩张器术后出现感染,行抗感染治疗后继续扩张;2例在皮瓣转移术后24 h内出现血肿,行血肿清除术.13例患儿共计15个扩张皮瓣,均顺利转移至颈部,效果良好.结论 预扩张后颈横皮瓣是儿童颈部病损修复的一种较好方法.

Abstract

Objective To explore clinical effects of using the pre-expanded flap from the cervical cutaneous branch of the transverse cervical artery to repair cervical scar contracture in children.Methods The feasibility and advantages of pre-expanded flap from the cervical cutaneous branch of the transverse cervical artery were analyzed by reviewing the pediatric patients with cervical scar contracture treated in the Department of Plastic Surgery,Xijing Hospital,Air Force Medical University from January 2009 to December 2019.Results Among the 13 cases,1 case underwent debridement and expander replacement because of expander exposure caused by poor incision healing after expander implantation;1 case continued to expand after anti-infection treatment because of infection after expander implantation;2 cases underwent hematoma evacuation within 24 h after flap transposition.A total of 15 expanded flaps were successfully transferred to the neck in all 13 cases with good results.Conclusion Pre-expanded flap from the cervical cutaneous branch of the transverse cervical artery is a good method for repairing cervical scar contracture in children.
